摘要
目的分析丁苯酞、尤瑞克林联合阿加曲班治疗急性缺血性脑卒中(AIS)的效果。方法抽取焦作市人民医院2018年1月至2020年12月收治的104例AIS患者,按随机数字表法分为三联组和二联组,每组52例。二联组采用尤瑞克林联合阿加曲班治疗,三联组在此基础上加用丁苯酞治疗,比较两组疗效、血清炎性因子水平、脑血流动力学指标、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、日常生活能力(Barthel指数)及不良反应发生情况。结果三联组总有效率(96.15%,50/52)高于二联组(78.84%,41/52),P<0.05;治疗后,三联组血清炎性因子水平低于二联组(P<0.05),脑血流动力学水平高于二联组(P<0.05);治疗后,三联组NIHSS评分低于二联组、Barthel指数高于二联组(P<0.05);三联组不良反应发生率(1.92%,1/52)低于二联组(15.38%,8/52),P<0.05。结论采用丁苯酞联合尤瑞克林、阿加曲班治疗AIS,可提升治疗效果,改善机体炎症反应和脑血流动力学,缓解患者神经功能受损情况,促进机体康复,且安全性良好。
Objective To analyze the efficacy of butylphthalide,urinary kallikrein and argatroban on acute ischemic stroke(AIS).Methods A total of 104 AIS patients treated in Jiaozuo People’s Hospital from January 2018 to December 2020 were selected and divided into triplet group and bigeminy group by random number table method,with 52 cases in each group.The bigeminy group was treated with urinary kallidinogenase and argatroban,and the triplet group was treated with butylphthalide based on the treatment of bigeminy group.The curative effect,serum inflammatory factors,cerebral hemodynamics,nerve function assessed by National Institutes of Health Stroke Scale(NIHSS)score,activities of daily living assessed by Barthel index,and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the triplet group was 96.15%(50/52),higher than the 78.84%(41/52)of the bigeminy group(P<0.05).After treatment,the levels of serum inflammatory factors in the triplet group were lower than those in the bigeminy group(P<0.05),and the cerebral hemodynamics were higher than that in the bigeminy group(P<0.05).After treatment,the triplet group had lower NIHSS score and higher Barthel index,compared with those in the bigeminy group(P<0.05).The incidence of adverse reactions in the triplet group(1.92%,1/52)was lower than that in the bigeminy group(15.38%,8/52),P<0.05.Conclusions Butylphthalide combined with urinary kallikrein and argatroban in the treatment of AIS can significantly improve the therapeutic effect,improve the inflammatory response and cerebral hemodynamics,relieve the neurological impairment of patients,and promote the recovery,with good safety.
