摘要
目的分析慢性心力衰竭患者(CHF)抑郁、焦虑状况及其影响因素。方法CHF患者60例,根据SDS和SAS评分将患者分为抑郁自评量表(SDS)评分≥53分组(n=18)、SDS评分<53分组(n=42)和焦虑自评量表(SAS)评分≥50分组(n=26)、SAS评分<50分组(n=34);采用本院自制问卷调查表收集患者一般资料,采用SDS评分量表评估患者抑郁情况,SAS量表评估患者焦虑情况;对CHF患者发生抑郁和焦虑进行单因素及logistic二元回归分析。结果SDS评分≥53分与SDS评分<53分CHF患者在NYHA分级、吸烟史、合并高血压、合并2型糖尿病、合并心律失常、月收入、婚姻状况、教育程度和病程方面比较,差异有统计学意义(P<0.05),其中婚姻状况、合并心律失常、合并2型糖尿病、合并高血压和NYHA分级是CHF发生的影响因素;SAS评分≥50分与SAS评分<50分的CHF患者在性别、年龄、教育程度、月收入状况、合并心律失常、合并2型糖尿病、合并高血压以及NYHA分级方面比较,差异有统计学意义(P<0.05),其中合并心律失常、合并2型糖尿病以及NYHA分级对CHF患者焦虑具有影响作用,月收入状况是CHF患者焦虑的保护因素。结论CHF患者的抑郁、焦虑状况与合并其他疾病、月收入和NYHA分级等因素密切相关。
Objective To investigate the status and influencing factors of depression and anxiety in patients with chronic heart failure(CHF).Methods According to SDS and SAS scores,60 patients with CHF were divided into SDS score≥53 group(n=18),SDS score<53 group(n=42),SAS score≥50 group(n=26),and SAS score<50 group(n=34).The general data of patients were collected by hospital-made questionnaires.Depression of patients was assessed by SDS and anxiety,by SAS.Depression and anxiety of patients with CHF were analyzed by univariate and logistic binary regression analysis.Results The differences in specific items(NYHA grading,smoking history,hypertension,Type 2 diabetes,arrhythmia,monthly income,marital status,education level,and course of disease)were compared between patients with SDS score≥53 and those with<53,differences were statistically significant(P<0.05).The marital status,arrhythmia,Type 2 diabetes,hypertension,and NYHA grading were influencing factors of CHF.Differences in gender,age,education level,monthly income,arrhythmia,Type 2 diabetes,hypertension,and NYHA grading between patients with SAS score≥50 and those with SAS score<50 were statistically significant(P<0.05).Aarrhythmia,Typ 2 diabetes and NYHA grading were of significant effects on anxiety,and monthly income was a protective factor for anxiety of CHF patients.Conclusion Depression and anxiety are closely related to other diseases,monthly income and NYHA grading in CHF patients.
