糖尿病早期肾损伤的相关因素及与亚临床甲减的关系分析 被引量:2

摘要 目的探究糖尿病早期肾损伤的相关因素及与亚临床甲状腺功能减退(SCH)的关系。方法回顾性分析2019年12月至2020年10月于本院接受治疗的62例2型糖尿病(T2DM)患者的临床资料,根据肾功能实验室检查结果分组,尿微量白蛋白(mALB)≤23 mg/L为无损伤组(n=35),mALB>23 mg/L为损伤组(n=27)。比较两组临床资料、肾功能、甲状腺功能及SCH发生率,Logistic回归分析T2DM患者早期肾损伤的危险因素。结果两组性别、高血压发病率、高血脂家族史、血清尿素氮(BUN)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)比较差异无统计学意义;两组年龄、冠心病发病率、糖尿病肾病家族疾病史、尿α1-微球蛋白(α1-MG)、尿β2-微球蛋白(β2-MG)、尿白蛋白肌酐比值(ACR)、促甲状腺激素(TSH)、SCH发病率比较差异有统计学意义(P<0.05);Logistic回归分析显示,尿α1-MG、尿β2-MG、尿ACR、TSH、SCH均是T2DM患者早期肾损伤的高危因素(P<0.05)。结论尿α1-MG、尿β2-MG、尿ACR、TSH、SCH均为T2DM患者早期肾损伤的关键影响因素,而TSH水平、SCH高发病率与其密切相关。 Objective To explore the related factors of early renal injury in diabetes and its relationship with subclinical hypothyroidism(SCH).Methods The clinical data of 62 patients with type 2 diabetes mellitus(T2DM)who were treated in our hospital from December 2019 to October 2020 were retrospectively analysized.According to the laboratory test results of renal function,the urinary microalbumin(mALB)≤23 mg/L as the non-invasive group(n=35),and the mALB>23 mg/L as the injured group(n=27).The clinical data,renal function,thyroid function and the incidence of SCH were compared between the two groups.The risk factors of early renal injury in T2DM patients were analyzed by logistic regression.Results There was no significant difference in sex,hypertension incidence rate,family history of hyperlipidemia,blood urea nitrogen(BUN),free triiodothyronine(FT3)and free thyroxine(FT4)between the two groups.Age,coronary heart disease incidence rate,family disease history of diabetes nephropathy,urineα1-microglobulin(α1-MG),β2-microglobulin(β2-MG),urinary albumin creatinine ratio(ACR),thyroid stimulating hormone(TSH)and the incidence rate of SCH were significantly different between two groups(P<0.05).Logistic regression analysis showed that urineα1-MG,β2-MG,ACR,TSH and SCH were high risk factors for early renal injury in T2DM patients(P<0.05).Conclusion Urineα1-MG,β2-MG,ACR,TSH and SCH are the key factors affecting early renal injury in T2DM patients,and the level of TSH and the high incidence rate of SCH are closely related to them.
