摘要
目的报道1例冻融胚胎移植术(FET)后发生的双侧卵巢妊娠(OP)病例,回顾并总结OP的病因,探讨其发病机制及高危因素,归纳术前诊断要点及当前主要治疗方式。以此提高临床对卵巢异位妊娠的认识。方法回顾性分析本院1例冷冻周期胚胎移植术后双侧卵巢妊娠患者的病历资料及诊疗经过,通过学习既往文献,总结卵巢异位妊娠发生的病因、危险因素、临床特点、超声诊断要点及鉴别诊断、治疗方式。结果1例28岁原发不孕女性,冷冻周期胚胎移植术后,双侧卵巢妊娠行双侧卵巢异位妊娠病灶切除术,半年内成功自然妊娠。结论辅助生殖技术(ART)后卵巢异位妊娠发生率高,通过血人绒毛膜促性腺激素(HCG)及经阴道超声能较早、较准确诊断卵巢异位妊娠,明确与其他病变包块的鉴别诊断,早期、准确诊断OP,尽早地为临床提供诊疗依据。早期手术治疗有利于保留患者的生育能力,避免出现危机患者生命的严重并发症,提高临床预后。
Objective 1 case of bilateral ovarian pregnancy(OP)after frozen-thawed embryo transfer(FET)was reported.The etiology of OP is reviewed and summarized,and its pathogenesis and risk factors are discussed.The preoperative diagnosis and main treatment methods are summarized to improve the clinical understanding of ovarian pregnancy.Methods Retrospectively analyzed the medical records and treatment of 1 patient with bilateral ovarian pregnancy after frozen-thawed embryo transfer in our hospital,and summarized the etiology,risk factors,clinical features,ultrasound diagnostic points and differential diagnosis,and treatment modalities for the occurrence of ovarian ectopic pregnancy by studying previous literature.Results A 28-year-old female with primary infertility had a successful natural pregnancy within 6 months after bilateral ovarian ectopic pregnancy resection following frozen-thawed embryo transfer.Conclusion The incidence of ectopic ovarian pregnancy after assisted reproductive technology(ART)is high.Through human chorionic gonadotropin(hCG)and transvaginal ultrasound,ectopic ovarian pregnancy can be diagnosed earlier and more accurately,clarify the differential diagnosis with other lesions of the mass,diagnose OP early and accurately,and provide clinical basis for treatment as early as possible.Early surgical treatment is beneficial to preserve fertility of patients,avoid serious complications of life in crisis patients,and improve clinical prognosis.
