摘要
目的探讨尿常规亚硝酸盐(U-NIT)、白细胞酯酶(LE)检测及细菌计数在尿路感染(UTI)中的诊断价值。方法选取2018年5月—2020年4月辽宁省残疾人服务中心100例疑似尿路感染患者,均行U-NIT、LE、检测及细菌计数检查,以细菌培养鉴定结果为"金标准",统计U-NIT、LE检测及细菌计数诊断的诊断结果和诊断效能。结果100例尿液标本中,共培养出阳性32例,占比32.00%,阴性68例,占比68.00%;32例阳性标本中,革兰阴性菌17株,占比53.13%(17/32),以大肠埃希菌25.00%(8/32)为主;革兰阳性菌15株,占比46.88%(15/32),以鸟肠球菌18.75%(6/32)、粪肠球菌12.50%(4/32)为主;100份尿液标本中,采用U-NIT诊断出阳性11例,阴性65例;采用LE诊断出阳性25例,阴性41例;采用细菌计数诊断出阳性27例,阴性50例;细菌计数诊断灵敏度84.38%(27/32)较LE诊断78.13%(25/32)、U-NIT诊断34.38%(11/32)高,U-NIT诊断特异度95.59%(65/68)较LE诊断60.29%(41/68)、细菌计数诊断73.53%(50/68)高(P<0.05)。结论在诊断UTI中U-NIT具有较高特异度,LE、细菌计数具有较高灵敏度,三者均可作为早期筛查UTI的手段,临床可根据实际情况将三者联合检测与细菌培养结果相结合,获取最佳诊断数据。
Objective To explore the diagnostic value of urine routine nitrite(U-NIT),leukocyte esterase(LE)detection and the detection of bacterial count in urinary tract infection(UTI).Methods Selected 100 patients with suspected urinary tract infection in April 2020 in May 2018,and U-NIT,LE,test and bacterial count test were performed.Taking bacterial culture identification results as"gold standard",the diagnostic results and efficacy of U-NIT,LE detection and bacterial count diagnosis were counted.Results Of the 100 urine specimens,A total of 32 positive cases were produced,For 32.00%,Negative for 68 cases,accounting for 68.00%;Of the 32 positive specimens,Seventeen Gram-negative bacteria,For 53.13%(17/32),Escherichia coli is mainly 25.00%(8/32);15 Gram-positive strains,For 46.88%(15/32),Mainly with 18.75%(6/32)and 12.50%(4/32);Of the 100 urine specimens,11 positive cases were diagnosed with U-NIT,Negative for 65 cases;The 25 cases of positive cases were diagnosed with LE,Negative for 41 cases;Positive 27 cases were diagnosed with bacterial counts,Negative for 50 cases;Bacterial count diagnostic sensitivity 84.38%(27/32)is higher than LE diagnosis 78.13%(25/32)and U-NIT diagnosis 34.38%(11/32),The U-NIT diagnostic specificity of 95.59%(65/68)is higher than 60.29%(41/68)for LE diagnosis and 73.53%(50/8)(P<0.05).Conclusion U-NIT has high specificity in diagnosis of UTI and LE,bacterial count has high sensitivity in diagnosis.All three can be used as early screening of UTI and can be combined with bacterial culture results according to the actual situation to obtain the best diagnostic data.
