尿路感染疾病中尿常规与尿培养检查指标相关性分析

摘要 目的:分析尿路感染疾病检查中尿常规与尿培养的关系。方法:纳入2020年10月至2023年6月前来我院就诊的疑似尿路感染患者送检的样本400份,先进行尿培养,剩余进行尿常规(尿干化+尿沉渣)检查,主要分析尿常规中白细胞酯酶(LEU)、白细胞(WBC)、亚硝酸盐(NIT)、细菌(BACT)检查结果及尿培养结果,观察不同白细胞酯酶、白细胞、亚硝酸盐、细菌等阳性结果时尿培养结果的阳性情况。结果:此次400份标本培养中,11份培养出三种及三种以上菌落,判为污染,排除出组。389份有效标本,共182份标本为尿培养阳性,培养阳性率46.79%;阳性样本中大肠埃希菌株数占比最高,达48.66%;尿常规检查中LEU阳性预测率为45.86%,阴性预测率为76.29%;NIT阳性预测率为86.11%,阴性预测率为77.94%;WBC阳性预测率53.55%,阴性预测率为71.96%;BACT阳性预测率47.41%,阴性预测率为88.24%;当NIT与WBC联合时,组合1(WBC阳性NIT阴性),尿培养阳性率40.20%;组合2(WBC阴性NIT阳性),尿培养阳性率80.00%;组合3(WBC及NIT共阳性),尿培养阳性率88.46%;组合4(WBC与NIT共阴性),尿培养阳性率9.09%。组合3(WBC及NIT共阳性)时,尿培养阳性率最高,达88.46%,WBC及NIT共阴性时,阳性率最低,为9.09%,不同WBC、NIT检查结果的尿培养阳性率有显著差异(P<0.05)。结论:尿路感染疾病诊断中,尿常规指标LEU、NIT、WBC、BACT预测率均不同,NIT阳性预测率较高,BACT阴性预测率较高,当WBC及NIT共阳性时尿培养阳性率为最高,因此,在尿路感染疾病诊治工作中,关注白细胞酯酶、白细胞、亚硝酸盐、细菌这四项指标,甚至将这些指标联合分析,意义显著。因此,通过尿常规检查能快速初筛出尿路感染疾病,尿常规检查优势突出,具有重要的临床价值。 Objective:To analyze the relationship between urine routine and urine culture in the examination of urinary tract infection.How:400 samples submitted by suspected patients with urinary tract infection who came to our hospital from October 2020 to June 2023 were included.Urine culture was performed first,and urine routine(urine drying+urine sediment)examination was performed for the rest.The results of LEucocyte esterase(LEU),leucocyte(WBC),nitrite(NIT),and bacteria(BACT)in urine routine were analyzed,and the positive results of urine culture were observed when different positive results of leucocyte esterase,leucocyte,nitrite,and bacteria were detected.Results:Among the 400 samples,11 cultured three or more kinds of colonies,which were deemed as contamination and excluded from the group.Of 389 effective samples,182 samples were positive for urine culture,with a positive rate of 46.79%.Escherichia coli strains accounted for 48.66%of the positive samples.The positive prediction rate of LEU was 45.86%,and the negative prediction rate was 76.29%.The positive prediction rate of NIT was 86.11%,and the negative prediction rate was 77.94%.The positive prediction rate of WBC was 53.55%,and the negative prediction rate was 71.96%.The positive prediction rate of BACT was 47.41%,and the negative prediction rate was 88.24%.When NIT and WBC were combined,the positive rate of urine culture in combination 1(WBC positive and NIT negative)was 40.20%.Combination 2(WBC negative NIT positive),urine culture positive rate of 80.00%;Combination 3(WBC and NIT positive),urine culture positive rate 88.46%;Combination 4(WBC and NIT negative),urine culture positive rate 9.09%.The positive rate of urine culture in combination 3(WBC and NIT co-positive)was the highest(88.46%),and the positive rate of urine culture in combination 3(WBC and NIT co-positive)was the lowest(9.09%).The positive rate of urine culture in different WBC and NIT test results was significantly different(P<0.05).Conclusion:In the diagnosis of urinary tract infection,the pr
