摘要
目的:系统评价初乳口腔免疫疗法对改善早产儿胃肠道喂养的效果,为初乳口腔免疫疗法的临床应用提供参考依据。方法:计算机检索PubMed、the Cochrane Library、Web of Science、Up To Date、中国知网、万方数据库、维普数据库中有关早产儿初乳口腔免疫疗法的随机对照试验研究,检索时间为建库至2021年5月31日,根据纳入及排除标准对文献进行初筛,并由2名研究员独立对筛选后的文献进行质量评价。采用RevMan 5.4软件进行Meta分析。结果:共纳入13篇随机对照试验进行研究。Meta分析结果显示,初乳口腔免疫疗法可降低新生儿坏死性小肠结肠炎发生率[RR=0.42,95%CI(0.26,0.68),P=0.0004],缩短达全肠道喂养时间[MD=-2.46,95%CI(-3.46,-1.45),P<0.00001],降低喂养不耐受发生率[RR=0.48,95%CI(0.36,0.64),P<0.00001]。结论:现有证据表明,初乳口腔免疫疗法可有效降低早产儿新生儿坏死性小肠结肠炎和喂养不耐受的发生率,缩短早产儿达全肠道喂养时间。
Objective:To systematically evaluate the effects of colostrum oral immunotherapy on gastrointestinal function of premature infants,and to provide reference basis for the clinical application of colostrum oral immunotherapy.Methods:RCTs on colostrum oral immunotherapy for premature infants were searched from PubMed,the Cochrane Library,Web of Science,Up To Date,CNKI,WanFang Database and VIP databases.The retrieval time was from the inception to May 31,2021,The literatures were screened according to the inclusion and exclusion criteria,and two researchers independently evaluated the quality of the screened literature.RevMan 5.4 software was used for Meta-analysis.Results:A total of 13 randomized controlled trials were included.Meta-analysis results showed that colostrum oral immunotherapy can reduce the incidence of neonatal necrotizing enterocolitis[RR=0.42,95%CI(0.26,0.68),P=0.0004],shorten the time of total intestinal feeding[MD=-2.46,95%CI(-3.46,-1.45),P<0.00001],and reduce the incidence of feeding intolerance[RR=0.48,95%CI(0.36,0.64),P<0.00001].Conclusions:Current evidence shows that colostrum oral immunotherapy can effectively reduce the incidence of neonatal necrotizing enterocolitis and feeding intolerance in preterm infants,and shorten the time for preterm infants to reach total intestinal feeding.
