摘要
目的:探究内热针疗法对膝骨性关节炎(knee osteoarthritis,KOA)兔软骨下骨成骨细胞凋亡的影响及其作用机制。方法:将30只新西兰兔按随机数字表法分为空白组10只、造模组20只,采用Hulth法制备兔KOA模型,造模成功后再随机分为模型组和治疗组各10只,治疗组进行4周的内热针治疗。干预前后分别采用改良Lequesen MG量表对实验兔进行行为学评价,采用苏木精-伊红染色法(hematoxylin-eosin staining,HE)和原位末端转移酶标记(TdT-mediated dUTP Nick-End Labeling,TUNEL)染色法观察各组实验兔膝关节软骨下骨组织病理学变化及成骨细胞的凋亡情况,蛋白免疫印迹法和实时荧光定量PCR分别检测软骨下骨中死亡因子(factor associated suicide,Fas)和Fas配体(fas ligand,FasL)的蛋白及mRNA的表达水平。结果:与空白组比较,模型组实验兔的行为学评分明显升高(P<0.05);与模型组比较,内热针能显著降低行为学评分(P<0.05)及改善实验兔膝关节软骨下骨组织的病理学变化,下调实验兔膝关节软骨下骨中Fas和FasL的蛋白及mRNA表达水平(P<0.05),且能显著抑制实验兔膝骨关节软骨下骨中成骨细胞的凋亡。结论:内热针疗法能显著改善KOA实验兔的组织病理学变化,抑制KOA中软骨下骨成骨细胞的凋亡,这可能与其抑制Fas和FasL的表达密切相关。
Objective:To explore the effect and mechanism of internal heating acupuncture on osteoblast apoptosis in subchondral bone of knee osteoarthritis(KOA)rabbits.Methods:Thirty New Zealand rabbits were randomly divided into blank group(n=10)and model group(n=20).Rabbit KOA model was prepared by Hulth method.After successful establishment of the model,the rabbits were randomly divided into model group(n=10)and treatment group(n=10).After 4 weeks of treatment,behavioral evaluation was performed using the modified Lequesen MG scale;Hematoxylin-eosin staining(HE)and TDT-mediated dutp nick-end labeling(TUNEL)were used to observe the pathological changes of subchondral bone and the apoptosis of osteoblasts in experimental rabbits.Western blot and Real-time quantitative PCR were used to detect the protein and mRNA expression of factor associated suicide(Fas)and fas ligand(FasL)in subchondral bone.Results:Compared with the blank group,the behavioral scores of the experimental rabbits in the model group were significantly higher(P<0.05).Compared with the model group,internal heat acupuncture could significantly reduce the behavioral score(P<0.05),improve the pathological changes of knee subchondral bone,down-regulate the protein and mRNA expression levels of Fas/FasL in experimental rabbit knee subchondral bone(P<0.05),and significantly inhibit the apoptosis of osteoblasts in experimental rabbit knee joint subchondral bone.Conclusion:Internal hot needle therapy can significantly improve the histopathological changes of rabbits in KOA and inhibit the apoptosis of subchondral osteoblasts in KOA,which may be closely related to its inhibition of the expression of Fas and FasL.
