摘要
目的探讨当归苦参丸联合透明质酸凝胶外用治疗脂溢性皮炎的效果。方法选择2017年12月至2021年12月北京医院收治的120例脂溢性皮炎患者,按照随机数字表法分为对照组和观察组,每组60例。对照组给予透明质酸凝胶外用治疗,观察组在对照组基础上给予当归苦参丸治疗,2组均治疗1个月。比较2组患者的中医证候评分、临床疗效、皮肤屏障功能以及皮肤表皮组织炎性因子水平。结果观察组治疗后的皮损数量形态、分布范围、鳞屑程度、渗出糜烂程度、瘙痒程度评分均低于对照组[(0.90±0.15)分比(1.21±0.13)分、(0.86±0.11)分比(1.37±0.17)分、(0.33±0.10)分比(0.81±0.13)分、(0.31±0.11)分比(0.76±0.14)分、(0.74±0.14)分比(1.13±0.19)分],差异均有统计学意义(均P<0.05)。观察组治疗后总有效率高于对照组[93.3%(56/60)比80.0%(48/60)],差异有统计学意义(χ^(2)=4.615,P=0.032)。观察组治疗后经表皮失水率、皮脂分泌率均低于对照组,皮肤含水量高于对照组[(9.0±1.3)%比(11.3±1.8)%、(53±6)%比(45±6)%、(24±4)%比(38±5)%],差异均有统计学意义(均P<0.05)。观察组治疗后的白细胞介素10水平高于对照组,γ干扰素、肿瘤坏死因子α水平均低于对照组,差异均有统计学意义(均P<0.05)。2组治疗期间均未出现明显不良反应。结论当归苦参丸联合透明质酸凝胶外用治疗脂溢性皮炎的效果良好,可有效缓解患者的临床症状,且可改善皮肤屏障功能与皮肤表皮组织炎症反应。
Objective To investigate the effect of Danggui Kushen pill combined with hyaluronic acid gel on seborrheic dermatitis.Methods One hundred and twenty patients with seborrheic dermatitis admitted to Beijing Hospital from December 2017 to December 2021 were enrolled.Patients were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with hyaluronic acid gel for external use,and the observation group was treated with Danggui Kushen pill based on the control group.Both groups were treated for 1 month.The traditional Cinese medicine symptom score,clinical efficacy,skin barrier function and levels of inflammatory factors in skin epidermis were compared between the two groups.Results After treatment,the scores of number,shape,distribution range of skin lesions,degree of scurf,degree of exudation and erosion and degree of pruritus in the observation group were lower than those in the control group[(0.90±0.15)vs(1.21±0.13),(0.86±0.11)vs(1.37±0.17),(0.33±0.10)vs(0.81±0.13),(0.31±0.11)vs(0.76±0.14),(0.74±0.14)vs(1.13±0.19)](all P<0.05).The total effective rate of the observation group was higher than that of the control group[93.3%(56/60)vs 80.0%(48/60)](χ^(2)=4.615,P=0.032).The transepidermal water loss rate and sebum secretion rate of the observation group after treatment were lower than those of the control group,and the skin water content of the observation group was higher than that of the control group[(9.0±1.3)%vs(11.3±1.8)%,(53±6)%vs(45±6)%,(24±4)%vs(38±5)%](all P<0.05).The level of interleukin-10 of the observation group after treatment was higher than that of the control group,and levels fo interferon-γand tumor necrosis factor-αwere lower than those of the control group(all P<0.05).There were no obvious adverse reactions in both groups during treatment.Conclusions Danggui Kushen pill combined with hyaluronic acid gel is effective on the treatment of seborrheic dermatitis.It can effectively relieve the clinical symptoms and improve the
