河北省某大学一起诺如病毒急性胃肠炎疫情的流行病学调查 被引量:4

Epidemiological investigation of an acute Norovirus gastroenteritis epidemic in a university in Hebei

摘要 目的了解2020年河北省某大学一起诺如病毒急性胃肠炎疫情的流行特点及暴发原因。方法采用现场流行病学方法,收集2020年10月31日河北省发生诺如病毒急性胃肠炎疫情的大学的基本情况,在全体学生及教职工中搜索病例并进行回顾性问卷调查,描述疾病流行特征,开展危险因素探索,同时采集病例肛拭子、食物和水等环境样本进行可疑致病菌检测。结果2020年河北省某大学诺如病毒急性胃肠炎疫情共发现病例24例,罹患率0.074%。其中男生5例、女生19例,男女性别比为1∶3.8。病例症状以发热(95.83%)、呕吐(91.67%)、恶心(91.67%)为主,无重症死亡病例。采集24例病例肛拭子、呕吐物样本共25份,22例密切接触者肛拭子样本22份。经诺如病毒核酸检测,22例病例肛拭子、1例病例呕吐物样本诺如病毒GⅡ型阳性,密切接触者均为阴性。共采集44名厨师及1号餐厅27名工作人员肛拭子样本71份,1名厨师肛拭子样本诺如病毒GⅡ型阳性,基因组测序显示为诺如病毒GⅡP16型。结论该疫情为一起由餐厅厨师感染引起的诺如病毒GⅡ型急性胃肠炎疫情。 Objective To learn about the epidemiological characteristics and causes of an outbreak of Norovirus acute gastroenteritis at a university in Hebei Provinve in 2020.Methods Field epidemiology methods were used to collected the basic information of the universities in Hebei Province where an outbreak of Norovirus occured on 31,October,2020,retrospective questionnaire survey was conducted in all students and staff,followed by descriptive analysis.Risk factors related to the epidemic were explored through cross-sectional and case-control study.While collecting environment samples such as anal swabs,food and water from caeses for testing of suspected pathogenic bacteria.Results A total of 24 cases were detected in a university in Hebei Province in 2020,with a prevalence rate of 0.074%.Among them,5 cases were males and 19 cases were females,with a male-female sex ratio of 1∶3.8.Sysptoms were mainly fever(95.83%),vomiting(91.67%),and nausea(91.67%)with no serious cases of death.A total of 25 anal swabs and vomits samples were collected from 24 cases and 22 anal swabs from 22 close contacts,in which,22 anal swabs,and 1 case were positive for Norovirus GⅡby Norovirus nucleic acid testing,and close contacts were negative.A total of 71 anal swabs from 44 chefs and 27 staff members of Restaurant 1.One chef′s anal swab was positive for Norovirus GⅡ,and genome sequencing showed that it was Norovirus GⅡP16.Conclusion This is an outbreak of Norovirus GⅡacute gastroenteritis caused by an infection in a restaurant chef.
