摘要
目的 分析磁共振成像(MRI)和电子计算机断层扫描(CT)检查在高胆红素血症患儿脑损伤诊断中的应用价值。方法 选取2019年11月—2020年11月永康市妇幼保健院收治的80例高胆红素血症患儿为研究对象,经脑干听觉诱发电位(BAEP)检测,分为正常组39例和异常组41例,均进行MRI和CT检查,比较两组患儿血清中总胆红素(TSB)、结合胆红素(CB)、间接胆红素(UCB)、总胆红素及白蛋白(ALB)水平,分析CT、MRI及两者联合对两组患儿脑损伤的诊断价值。结果 异常组患儿TSB、CB、UCB及ALB水平[(475.31±42.39)μmol/L、(435.76±40.38)μmol/L、(33.73±12.16)μmol/L及(0.85±0.14)g/L]均明显高于正常组[(449.38±40.62)μmol/L、(409.25±39.32)μmol/L、(25.24±11.57)μmol/L及(0.70±0.16)g/L],差异均有统计学意义(t=2.791、2.973、3.196及4.468,均P<0.05)。MRI检查灵敏度、特异度及准确率均明显高于CT检查,误检率明显低于CT检查;两者联合检查的准确率明显高于单项检查,误检率明显低于单项检查。结论 MRI对高胆红素血症患儿脑损伤的检查准确性高于CT检查,两者联合应用的准确性高于单项检查,且误检率低。
Objective To analyze the value of magnetic resonance imaging(MRI) and computed tomography(CT) in the diagnosis of brain injury in children with hyperbilirubinemia. Methods A total of 80 children with hyperbilirubinemia admitted to Yongkang Maternal and Child Health Hospital from November 2019 to November 2020 were selected as the study subjects. According to the detection of brainstem auditory evoked potential(BAEP), they were divided into normal group(39 cases) and abnormal group(41 cases), and underwent MRI and CT examination. The serum levels of total bilirubin(TSB), conjugate bilirubin(CB), indirect bilirubin(UCB), total bilirubin and albumin(ALB) in the two groups were compared, and the diagnostic value of CT, MRI and their combination for brain injury in the two groups was analyzed. Results TSB, CB, UCB and ALB in abnormal group The levels of [(475.31±42.39) μmol/L,(435.76±40.38) μmol/L,(33.73±12.16) μmol/L and(0.85±0.14) g/L] were significantly higher than those of normal group [(449.38±40.62) μmol/L,(409.25±39.32) μmol/L,(25.24±11.57) μmol/L and(0.70±0.16) g/L], the differences were statistically significant(t=2.791, 2.973, 3.196 and 4.468, all P<0.05). The sensitivity, specificity and accuracy of MRI were significantly higher than that of CT, and the error detection rate was significantly lower than that of CT. The accuracy of the combined test was significantly higher than that of single test and the error rate was significantly lower than that of single test. Conclusion MRI was more accurate than CT in detecting brain injury in children with hyperbilirubinemia, and the combined application of MRI is more accurate than single examination, and the false detection rate was lower.
