摘要
目的系统评价经桂枝附子汤治疗类风湿关节炎(rheumatoid arthritis,RA)的疗效,运用基因表达数据库(Gene Expression Omnibus,GEO)联合网络药理学探讨其作用机制,并运用分子对接进行验证。方法检索维普数据库(VIP data)、中国知网(ChinaNational Knowledge Infrastructure,CNKI)、万方数据库(Wanfang data)、中国生物医学文献数据库(China Biology Medicine disc,CBM)、PubMed、考克兰图书馆循证医学数据库(the Cochrane Library)、荷兰医学文摘(Excerpta Medica Database,EMBASE)数据库建库至2020-09-01日。纳入研究均为随机对照试验(randomized controlled trial,RCT)。采用Revman 5.3软件进行统计分析,结局的效应指标为相对危险度(relative risk,RR)和加权均数差(weighted mean difference,WMD),以95%置信区间(confidence interval,CI)表示。通过GEO检索“Rheumatoid Arthritis”,物种限制为人,下载GSE56409芯片与平台数据,进行差异分析并得出差异基因。通过中医药系统药理学数据库分析平台(Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform,TCMSP)和Drugbank数据库检索药物相关信息。将两者结果互取交集,并进行拓扑网络构建及基因本体(Gene Ontology,GO)和京都基因和基因组百科全书(Kyoto Encyclopedia of Genes and Genomes,KEGG)富集分析。同时将获得的主要药物成分和相关通路基因靶蛋白进行分子对接。结果Meta分析结果显示,桂枝附子汤组在治疗有效率上优于对照组(RR=1.20,95%CI:1.12~1.29),在红细胞沉降率(erythrocyte sedimentation rate,ESR)和C反应蛋白(C-reaction protein,CRP)改善上分别优于对照组(WMD=-5.25,95%CI:-8.50~-2.00;WMD=-6.20,95%CI:-7.33~-5.08)。根据TCMSP获得祛除重复后的药物活性成分107个,槲皮素、β-谷甾醇、豆甾醇、儿茶素等为其主要成分。在Drugbank数据库中得到药物成分靶点2383个,经基因名转换后共获得桂枝附子汤药物靶点2055个。根据GEO获得RA差异基因953个。�
Objective To systematically evaluate the efficacy of Guizhi Fuzi decoction in the treatment of rheumatoid arthritis(RA),and to explore its mechanism by Gene Expression Omnibus(GEO)combined with network pharmacology,and conduct molecular docking verification.Methods VIP data,China National Knowledge Infrastructure(CNKI),Wanfang data,China Biology Medicine disc(CBM),PubMed,the Cochrane Library,the Excerpta Medica Database(EMBASE)were searched from the establishment of the databases to September 1,2020.All included studies were randomized controlled trials(RCT).Statistical analysis was conducted by Revman 5.3 software.The effect indicators of outcome were relative risk(RR)and weighted mean difference(WMD),which expressed by 95%confidence interval(CI)."Rheumatoid Arthritis"was searched through GEO database,and the species was restricted to human.GSE56409 chip and platform data were downloaded for differential analysis and differential genes were obtained.Drug related information was searched by Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP)and Drugbank database.Took the intersection of the two results,the topological network constructed,and enrichment analysis of Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)were performed.At the same time,the main drug components and related pathway gene target protein were molecular docking.Results Meta-analysis results showed that Guizhi Fuzi decoction group was superior than control group in treatment efficiency(RR=1.20,95%CI:1.12-1.29).Erythrocyte sedimentation rate(ESR)and C-reaction protein(CRP)were better than those in the control group(WMD=-5.25,95%CI:-8.50 to-2.00);(WMD=-6.20,95%CI:-7.33 to-5.08).According to TCMSP,107 active ingredients were obtained,including quercetin,β-sitosterol,stigmasterol and catechin.2383 drug component targets were obtained from Drugbank database,and 2055 drug targets were obtained after gene name conversion.A total of 953 RA differential genes were obtained from GEO.After the intersect
