摘要
目的:探讨微创远端胃癌手术与D2淋巴结清扫术联合治疗早期胃癌的疗效。方法:选取2019年1月至2020年1月在河南科技大学第一附属医院胃肠外科接受诊治的66例早期胃癌患者。根据随机抛硬币法分为观察组与对照组,各33例。对照组采用开腹远端胃癌手术结合D2淋巴结清扫术治疗,观察组采用微创远端胃癌手术与D2淋巴结清扫术联合治疗。本研究观察时间为1年,随访2年。比较两组患者手术相关指标、淋巴清除情况、术后并发症发生情况及术后不同时间段的复发情况。结果:在围手术期,观察组患者的手术出血量、手术时间、术后排气时间、术后住院时间、并发症总发生率均低于对照组,差异具有统计学意义(P<0.05)。两组患者的淋巴清除情况比较,差异无统计学意义(P>0.05)。术后的不同时间段,观察组患者的复发率与对照组比较,差异无统计学意义(P>0.05)。结论:微创远端胃癌手术与D2淋巴结清扫术联合治疗手术创伤小,术后患者恢复速度快,可减少并发症发生,缩短术后患者的住院时间。
Objective To investigate the efficacy of minimally invasive distal gastric cancer surgery combined with D2 lymph node dissection in the treatment of early gastric cancer.Methods A total of 66 patients with early gastric cancer treated in the Department of Gastrointestinal Surgery of the First Affiliated Hospital of Henan University of Science and Technology from January 2019 to January 2020 were selected.According to the random coin flipping method,they were divided into an observation group and a control group,with 33 cases in each group.The control group was treated with open distal gastric cancer surgery combined with D2 lymph node dissection,and the observation group was treated with minimally invasive distal gastric cancer surgery combined with D2 lymph node dissection.The study was observed for 1 year and followed up for 2 years.Operation related indicators,lymphatic clearance,postoperative complications and recurrence at different postoperative periods were compared between the two groups.Results In the perioperative period,the amount of surgical bleeding,operative time,postoperative exhaust time,postoperative hospital stay and total incidence of complications in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in lymphatic clearance between the two groups(P>0.05).There was no significant difference in recurrence rate between the observation group and the control group at different time after surgery(P>0.05).Conclusion Minimally invasive distal gastric cancer surgery combined with D2 lymph node dissection in the treatment of early gastric cancer has low of surgical trauma,fast postoperative recovery,reduced complications and shortened postoperative hospital stay.
