摘要
目前,中国门静脉高压症患者的数量不断增加。门静脉高压症引起的食管胃底静脉曲张破裂出血、瘀血性脾大、脾功能亢进、顽固性胸腔积液、顽固性腹腔积液是影响患者生存的高危因素。经颈静脉肝内门体分流术(TIPS)通过在肝静脉与门静脉之间的肝实质内建立分流道,以微创介入的方式降低门静脉阻力,是降低肝硬化患者门静脉压力的关键措施之一。但是,传统的TIPS手术在手术时间、并发症等方面存在较多问题。近年来,超声引导下穿刺门静脉及肝静脉作为一种新技术,能够辅助TIPS手术治疗门静脉高压症,可以缩短手术时间,减少辐射暴露,减轻患者的痛苦。
At present,the number of patients with portal hypertension in China is increasing.The rupture of esophageal and gastric variceal bleeding,congestive splenomegaly,hypersplenism,and refractory pleural and abdominal ascites caused by portal hypertension are factors that endanger the survival of patients.Transjugular intrahepatic portosystemic shunt(TIPS)is one of the key measures to reduce portal pressure in patients with liver cirrhosis by establishing a shunt in the liver parenchyma between the hepatic vein and the portal vein,and using minimally invasive intervention to significantly reduce the resistance of the portal vein.However,because traditional TIPS surgery has many problems in terms of operation time and complications,a new TIPS approach,the ultrasound-guided puncture of the portal vein and hepatic vein can assist TIPS in the portal hypertension,shorten the operation time,reduce radiation exposure and patient suffering.
