摘要
目的分析甘精胰岛素皮下注射对2型糖尿病伴上消化道出血患者急性期血糖的控制效果。方法选取2021年1—12月该院收治的T2DM伴上消化道出血患者共52例,依据随机数表法分成对照组(短效类胰岛素静注)和观察组(于对照组基础上加以甘精胰岛素皮下注射),各26例,比较两组血糖控制效果。结果观察组的有效率为92.31%高出对照组的69.23%,差异有统计学意义(P<0.05);治疗后,两组FPG、2 hPG水平均较治疗前降低,且观察组FPG、2 hPG水平低于对照组,差异有统计学意义(P<0.05);两组出血总量及大便潜血转阴时间相比,差异无统计学意义(P>0.05)。观察组低血糖、心慌、出汗和饥饿感等不良反应少于对照组,差异有统计学意义(P<0.05)。结论甘精胰岛素皮下注射对T2DM伴上消化道出血者急性期血糖控制效果理想,同时不良反应较少,安全性较高。
Objective To analyze the effect of subcutaneous injection of insulin glargine on blood glucose control in patients with type 2 diabetes mellitus and upper gastrointestinal bleeding in acute phase.Methods A total of 52 patients with T2DM with upper gastrointestinal bleeding treated in the hospital from January to December 2021 were randomly divided into control group(intravenous injection of short acting insulin)and observation group(subcutaneous injection of insulin glargine on the basis of the control group),with 26 cases in each group.The blood glucose control effects of the two groups were compared.Results The effective rate of the observation group was 92.31%,which was higher than that of the control group 69.23%,the difference was statistically significant(P<0.05).After treatment,the levels of FPG and 2 hPG in the two groups were lower than before treatment,and the levels of FPG and 2 hPG in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the total amount of bleeding and the time of stool occult blood turning negative between the two groups(P>0.05).The adverse reactions such as hypoglycemia,panic,sweating and hunger in the observation group were less than those in the control group,the difference was statistically significant(P<0.05).Conclusion Subcutaneous injection of insulin glargine has ideal effect on blood glucose control in acute phase of T2DM patients with upper gastrointestinal hemorrhage,with less side effects and higher safety.
