摘要
目的探讨在神经根型颈椎病中应用小针刀联合穴位注射法治疗的疗效及对患者颈椎活动度的影响。方法对河南省洛阳正骨医院收治的神经根型颈椎病患者的临床资料展开回顾性分析,患者共90例,就诊时间均为2018年7月至2020年7月;根据患者采用的治疗方案将其分为对照组、观察组。对照组纳入45例患者应用小针刀闭合性松解术联合常规针刺治疗,观察组纳入45例患者应用小针刀闭合性松解术联合穴位注射法治疗。对比两组患者治疗前后的疼痛视觉模拟评分法(VAS)评分、颈椎残障指数量表(NDI)评分、颈椎活动度(包括前屈、左旋、左侧屈、后伸、右旋、右侧屈)、临床疗效、上肢肌电图传导速度。结果治疗3个月后,观察组颈椎VAS、NDI评分较对照组低;观察组颈椎活动度高于对照组;观察组治疗有效率为93.33%,较对照组的75.56%高,组间对比,差异有统计学意义(P<0.05)。治疗后,两组上肢肌电图尺神经、正中神经传导速度高于治疗前,且观察组上述指标均高于对照组,差异均有统计学意义(P<0.05)。结论在神经根型颈椎病中应用小针刀联合穴位注射法治疗能够显著提高患者的治疗效果,可减轻颈椎疼痛,有效改善颈椎活动度,促进颈椎功能的恢复,提高神经传导速度。
Objective To investigate the effect of small needle knife combined with acupoint injection in the treatment of cervical radiculopathy and its influence on the range of motion of the cervical spine.Methods A retrospective analysis was carried out on the clinical data of patients with cervical radiculopathy in our hospital.A total of 90 patients were treated from July 2018 to July 2020,they were divided into control group and observation group according to the treatment plan adopted by the patients.The control group included 45 patients treated with small needle knife combined with conventional acupuncture,and the observation group included 45 patients treated with small needle knife combined with acupoint injection.Visual analogue scale(VAS)score of pain,cervical spine disability index(NDI)score,cervical spine mobility(including forward flexion,left rotation,left flexion,backward extension,right rotation,right flexion)before and after treatment in the two groups of patients,clinical efficacy,upper extremity EMG conduction velocity were compared between two groups.Results After 3 months of treatment,the cervical spine VAS and NDI scores in the observation group were lower than those in the control group;the cervical spine activity in the observation group was higher than that in the control group;the total effective rate of the treatment in the observation group was 93.33%,which was higher than 75.56%in the control group,the difference were statistically significant(P<0.05).After treatment,the conduction velocities of the ulnar nerve and median nerve in the upper limb EMG in the two groups were higher than those before treatment,and the above indexes in the observation group were higher than those in the control group,with statistical significance(P<0.05).Conclusion The application of small needle knife combined with acupoint injection in the treatment of cervical radiculopathy can significantly improve the therapeutic effect of patients,relieve cervical pain,effectively improve cervical mobility,promote the recove
