摘要
目的比较完全腹腔镜远端胃癌根治术(TLDG)和腹腔镜辅助下远端胃癌根治术(LADG)治疗胃癌的临床效果。方法抽取2016年7月至2020年7月濮阳市油田总医院收治的胃癌患者138例,按照手术方案将其分为TLDG组和LADG组,每组69例。LADG组予以LADG,TLDG组予以TLDG。比较两组围术期情况、胃肠功能指标(血清胃动素和胃泌素水平)、功能状态量表(KPS)评分、视觉模拟评法(VAS)评分、并发症、复发率。结果 TLDG组切口长度、肠蠕动恢复时间、失血量少于LADG组(P<0.05)。术后,TLDG组血清胃动素、胃泌素水平高于LADG组(P<0.05)。术后,TLDG组KPS评分高于LADG组(P<0.05)、VAS评分低于LADG组(P<0.05)。TLDG组并发症发生率(2.90%,2/69)低于LADG组(11.59%,8/69),P<0.05。两组复发率、中位生存期比较,差异均未见统计学意义(P均>0.05)。结论与LADG比较,TLDG治疗胃癌切口小,失血量少,术后肠蠕动和胃肠动力恢复快,术后疼痛程度较轻。
Objective To compare the clinical effects of total laparoscopic distal gastrectomy(TLDG)and laparoscopy-assisted distal gastrectomy(LADG)on gastric cancer.Methods A total of 138 gastric cancer patients admitted to Puyang Oilfield General Hospital from July 2016 to July 2020 were selected and divided into TLDG group and LADG group according to the operation plans,with 69 cases in each group.LADG group was given LADG,and TLDG group was given TLDG.The perioperative conditions,gastrointestinal function index(the levels of serum motilin and gastrin),functional status scale(KPS)score,visual analogue scale(VAS)score,complications and recurrence rates were compared between the two groups.Results The incision length,bowel movement recovery time and blood loss in the TLDG group were less than those in the LADG group(P<0.05).After operation,the levels of serum,motilin and gastrin in the TLDG group were higher than those in the LADG group(P<0.05).After operation,the KPS score in the TLDG group was higher than that in the LADG group(P<0.05),and the VAS score in the TLDG group was lower than that in the LADG group(P<0.05).The complication rate in TLDG group was 2.90%(2/69),which was lower than 11.59%(8/69)in LADG group(P<0.05).There was no significant difference in recurrence rate and median survival between the two groups(all P>0.05).Conclusions Compared with LADG,TLDG has a smaller incision and less blood loss in the treatment of gastric cancer,which can promote the postoperative recovery of peristalsis and gastrointestinal motility,and relieve pain.
