摘要
目的:总结血透通路相关肿胀手综合征CT血管成像(CTA)特征及其临床表现,探讨双源CT血管成像诊断肿胀手综合征的应用价值。方法:回顾性分析我院31例血透通路功能不良伴肿胀手患者的临床与CTA资料,分为中心静脉病变组与非中心静脉病变组,统计两组间在一般资料及临床表现的差异,对比分析两组CTA表现与临床特征相关性。结果:两组间在性别、年龄、内瘘使用年限等一般资料的差异均无统计学意义(P>0.05),中心静脉组(22例)CTA表现为血管畸形2例,管壁病变致狭窄闭塞14例、外压性狭窄5例,通路正常对侧中心静脉闭塞1例;非中心静脉组(9例)CTA表现:管壁病变致狭窄及闭塞(4例),外压性狭窄2例,皮肤过敏及移植物动静脉内瘘(AVG)至血清肿各1例,阴性(1例)。中心静脉组为全上肢肿胀及颈胸壁静脉曲张(10例),全上肢肿胀(7例),前臂(含手)肿胀(5例);非中心静脉组临床表现为前臂肿胀(3例)、手肿胀(4例)、2例全上肢肿胀。结论:CTA可作为诊断血透通路功相关肿胀手综合征常规影像学检查方法。
Purpose:To summarize the CT angiography(CTA)features and clinical manifestations of swelling hand syndrome,and to explore the application value of dual-source CT angiography in diagnosing swelling hand syndrome.Methods:The clinical data and CTA data of 31 patients with swelling hand syndrome during hemodialysis were collected.The patients were divided into central venous lesion group and non-central venous lesion group according to whether they had central venous lesions.The differences in general data and clinical manifestations between the two groups were statistically analyzed,and the correlation between CTA performance and clinical characteristics of the two groups was compared and analyzed.Results:There was no significant difference in the general data,such as gender,age,and service life of internal fistula between the two groups(all P>0.05).In the central venous lesion group(22 cases),CTA showed 2 cases of vascular malformation,14 cases of stenosis and occlusion caused by wall lesions,5 cases of stenosis caused by pressure,1 case of normal while central venous occlusion in other side.In non-central venous lesion group(9 cases),CTA showed 4 cases of stenosis and occlusion caused by wall lesions,2 cases of stenosis caused by pressure,1 case of skin allergy and seroma in arteriovenous graft(AVG)each,1 case with negative result.In the central venous lesion group,there were 10 cases of swelling of the whole upper limb and neck and chest wall varicose veins,7 cases of the swelling of the whole upper limb,5 cases of swelling of the forearm(including hands).In the non-central venous lesion group,there were 3 cases of swelling forearm(including hands),4 cases of hand swelling,2 cases of swelling upper limb.Conclusion:CTA can be used as a routine imaging method for the diagnosis of swelling hand.
