摘要
目的分析北京市某三级肿瘤专科医院胃癌患者在医药分开政策实施前后住院费用变化情况及住院费用的相关影响因素,为了解胃癌患者住院费用情况、减轻患者经济负担及优化医院运营管理提供数据支持。方法选取北京市某三级肿瘤专科医院2016—2018年2644位胃癌住院患者为研究对象,收集患者的基本信息、住院费用及其构成和疾病特征等信息,对比分析北京2017年4月医药分开政策实施前后住院费用的变化情况,通过多元回归模型探索住院费用的影响因素。结果实施医药分开政策后患者住院费用降低(p<0.05),药占比下降。多元回归分析结果提示患者的年龄、住院天数和医药分开政策是住院费用变化的影响因素(p<0.05)。结论医药分开政策实施后胃癌患者的住院费用明显降低。通过控制患者的住院天数、药占比和检查费占比的因素后,可以有效控制患者的住院费用,为医疗机构精细化运营管理提供数据支持。
Objective To analyze the changes of hospitalization expenses and related factors of hospitalization expenses for gastric cancer patients in a tertiary oncology hospital in Beijing before and after the implementation of the policy separating medical treatment and drug sales,in order to provide data support for understanding the hospitalization expenses of gastric cancer patients,reducing the economic burden of patients and optimizing hospital operation management.Methods 2644 inpatients with gastric cancer in a tertiary oncology hospital in Beijing from 2016 to 2018 were selected as the research objects,and the basic information,hospitalization expenses and their composition and disease characteristics of the patients were collected.The changes of hospitalization expenses before and after the implementation of the separation policy of medicines saling and treatment in Beijing in April 2017 were compared and analyzed,and the influencing factors of hospitalization expenses were explored through multiple regression model.Results After the implementation of the policy of separating medical treatment and drug sales,the hospitalization expenses of patients decreased(p<0.05),and the proportion of medicines decreased.The results of multiple regression analysis indicated that the patient’s age,the length of hospital stay and the policy of separating medical treatment and drug sales were the influencing factors of the changes in hospitalization expenses(p<0.05).Conclusion After the implementation of the policy of separating medical treatment and drug sales,the hospitalization expenses of gastric cancer patients have been effectively reduced.By controlling the factors such as the length of hospital stay,the proportion of drugs and the proportion of examination fees,the hospitalization expenses of patients can be effectively controlled.It provides data support for the fefined management of the operation of medical institutions.
