摘要
目的:探讨超声引导下针吸细胞学检查(ultrasonic guidance fine needle aspiration cytology,US-G FNAC)在甲状腺混合性回声结节(Thyroid mixed echo nodule)微波消融术前的诊断价值。方法:选取2018年8月~2019年8月于我院行微波消融术治疗的TNS患者54例,其中实性结节组22例,混合性结节组32例。所有患者在行微波消融术治疗前均行US-G FNAC,参照Bethesda标准判定检查结果。细胞学结果提示少量滤泡上皮细胞则视为基本满意标本,血液、胶质及淋巴细胞视为不满意标本,其余则视为满意标本。采用χ^(2)检验比较两组结节细胞学结果的满意率。结果:22个实性结节微波消融术前细胞学结果:良性滤泡结节14例(63.64%),桥本4例(18.18%),细胞不典型病变1例(4.55%),少量滤泡上皮细胞2例(9.09%),血液、胶质及淋巴细胞1例(4.55%);32个混合回声结节术前细胞学结果:良性滤泡结节20例(62.5%),桥本2例(6.25%),囊肿性病变2例(6.25%),少量滤泡上皮细胞5例(15.63%),血液、胶质及淋巴细胞3例(9.38%)。两组病例总体穿刺成功率为92.59%,两组病例满意标本、基本满意标本、不满意标本的χ;检验的P值>0.05。实性结节及混合性结节细胞学穿刺成功率没有差异性。结论:US-G FNAC在甲状腺混合性结节微波消融术前具有较高的诊断价值。
Objective To investigate the diagnostic value of ultrasound-guided fine needle aspiration cytology (US-G FNAC) before microwave ablation of thyroid mixed echo nodule.Methods 54 patients with TNS who underwent microwave ablation in our hospital from August 2018 to August 2019 were selected,including 22 cases in the solid nodule group and 32 cases in the mixed nodule group.All patients underwent us-g FNAC before microwave ablation,and the examination results were determined according to Bethesda standard.Cytological results showed that a small number of follicular epithelial cells were regarded as basically satisfactory specimens,blood,glia and lymphocytes were regarded as unsatisfactory specimens,and the rest were regarded as satisfactory specimens.The χ^(2) test was used to compare the satisfaction rate of the nodule cytology results between the two groups.Results The cytological results of 22 solid nodules before microwave ablation:benign follicular nodules in 14 cases (63.64%),Hashimoto in 4 cases (18.18%),atypical cellular lesions in 1 case (4.55%),a small number of follicular epithelial cells in 2 cases (9.09%),blood,glia and lymphocytes in 1 case (4.55%);Preoperative cytological results of 32 mixed echo nodules:benign follicular nodules in 20 cases (62.5%),Hashimoto in 2 cases (6.25%),cystic lesions in 2 cases (6.25%),a small amount of follicular epithelial cells in 5 cases (15.63%),blood,glia and lymphocytes in 3 cases (9.38%).The overall puncture success rate of the two groups was 92.59% χ;.The P value of the test is > 0.05.There was no difference in the success rate of cytological puncture between solid nodules and mixed nodules.Conclusion US-G FNAC has high diagnostic value before microwave ablation of mixed thyroid nodules.
