摘要
目的:观察并比较利伐沙班与依诺肝素在膝关节骨性关节炎(KOA)患者关节清理术后预防静脉血栓的有效性和安全性.方法:选取2019年10月~2021年2月在我院就诊的200例行关节清理术的KOA患者,根据随机数表法分为利伐沙班组和依诺肝素组,分别给予相应药物用于围术期抗凝治疗;比较组间膝关节疼痛评分(VAS)、功能评分(Lysholm)、膝关节活动度、膝关节最大屈曲度和平衡功能、术后7天静脉血栓发生率、D-二聚体水平及不良反应发生率等.结果:术后4周,两组患者的VAS评分均低于术后,Lysholm评分均高于术后,两组之间VAS评分和Lysholm评分比较,差异无统计学意义;术后1年,患者的关节活动度、最大屈曲度和BBS评分均大于术前,两组之间关节活动度、最大屈曲度和BBS评分比较,差异无统计学意义;利伐沙班组的静脉血栓发生率、D-二聚体水平及不良反应发生率均显著低于依诺肝素组.结论:与依诺肝素相比,利伐沙班在预防KOA患者关节清理术后静脉血栓方面更有积极意义,不良反应发生率更低,可为临床用药提供一定参考.
Objective To observe and compare the efficacy and safety of rivaroxaban and enoxaparin in preventing venous thrombosis after joint debridement in patients with knee osteoarthritis(KOA).Methods A total of 200 KOA patients who underwent joint debridement in our hospital from October 2019 to February 2021 were randomly divided into rivasaban group and enoxaparin group.Knee pain score(VAS),function score(Lysholm),range of motion of knee joint,maximum flexion and balance function of knee joint,incidence of venous thrombosis 7 days after operation,level of D-dimer and incidence of adverse reactions were compared between groups.Results At 4 weeks after operation,the VAS score of the two groups was lower than that after operation,and the Lysholm score was higher than that after operation.There was no significant difference between the two groups in VAS score and Lysholm score;One year after operation,the range of motion,maximum flexion and BBS scores of the patients were higher than those before operation.There was no significant difference between the two groups;The incidence of venous thrombosis,the level of D-dimer and the incidence of adverse reactions in rivaroxaban group were significantly lower than those in enoxaparin group.Conclusion Compared with enoxaparin,rivasaban has more positive significance in preventing venous thrombosis after joint debridement in patients with KOA,and the incidence of adverse reactions is lower,which can provide some reference for clinical medication.
