同型半胱氨酸联合β2微球蛋白对H型高血压患者肾损害的早期预测价值 被引量:6

Early predictive value of homocysteine combined withβ2 microglobulin for renal damage in patients with H type hypertension

摘要 目的探讨同型半胱氨酸(Hcy)与β2微球蛋白(β2-MG)联合预测H型高血压患者肾损害的应用价值。方法回顾性选取2020年6月—2021年6月本院收治的H型高血压患者100例作为研究对象,其中合并肾功能损伤者45例,肾功能正常者55例,合并肾功能损伤中高血压Ⅰ级11例,Ⅱ级14例,Ⅲ级20例。对比H型高血压肾功能损伤与肾功能正常患者、不同分级H型高血压合并肾损害患者之间血清Hcy及血、尿β2-MG水平,并对血清Hcy及血、尿β2-MG水平单独级联合对H型高血压肾损害早期预测、不同分级H型高血压合并肾损害早期预测价值进行分析。结果H型高血压肾功能损伤者血清Hcy及血、尿β2-MG水平均明显高于肾功能正常者(P<0.05);Ⅰ级~Ⅱ级H型高血压合并肾损伤患者血清Hcy及血、尿β2-MG水平明显低于Ⅲ级H型高血压合并肾损伤患者(P<0.05);ROC曲线分析结果显示:血清Hcy及血、尿β2-MG水平单独及联合对H型高血压肾损害早期预测价值高于各项指标单一检测(P<0.05);ROC曲线分析结果显示:血清Hcy及血、尿β2-MG水平单独及联合对不同分级H型高血压合并肾损害早期预测价值高于各项指标单一检测(P<0.05)。结论血清Hcy联合血、尿β2-MG可将各自诊断优势互补,有效提高对H型高血压肾损害早期预测准确性,且有利于判断高血压严重程度。 Objective To explore the application value of homocysteine(Hcy)andβ2 microglobulin(β2-MG)in predicting renal damage in patients with H type hypertension.Methods One hundred patients with H type hypertension admitted to our hospital from June 2020 to June 2021 were selected as the study objects,including 45 patients with renal impairment,55patients with normal renal function,11 patients with grade I hypertension,14 patients with grade II hypertension and 20 patients with grade III hypertension.The levels of serum Hcy,blood and urineβ2-MG were compared between patients with normal renal function and patients with different grades of H-type hypertension complicated with renal damage,and the value of serum Hcy,blood and urineβ2-MG in early prediction of renal damage in H-type hypertension and different grades of H-type hypertension complicated with renal damage was also analyzed.Results The levels of serum Hcy,β2-MG in blood and urine of patients with H-type hypertension with renal impairment were significantly higher than those with normal renal function(P<0.05).The levels of serum Hcy,β2-MG in blood and urine of patients with gradeⅠ-ⅡH hypertension complicated with kidney injury were significantly lower than those of patients with grade III H hypertension complicated with kidney injury(P<0.05).ROC curve analysis showed that serum Hcy,blood and urineβ2-MG levels alone or in combination had higher predictive value for early renal damage in H-type hypertension than single detection of each index(P<0.05).ROC curve analysis showed that serum Hcy,blood and urineβ2-MG levels alone or in combination had higher predictive value for different grades of H-type hypertension complicated with renal damage than single detection of each index(P<0.05).Conclusion Serum Hcy combined withβ2-MG in blood and urine can complement each other’s diagnostic advantages,effectively improve the accuracy of early prediction of renal damage in H type hypertension,and help to judge the severity of hypertension.
