摘要
目的 探讨外固定架治疗高风险老年患者股骨粗隆间骨折的疗效。方法 2014年6月至2019年3月回顾分析42例应用外固定架治疗股骨粗隆间骨折的患者。记录血红蛋白计数、手术时长、术中出血量、末次随访颈干角、骨折愈合时间及随访时间。末次随访时采用Harris髋关节评分评估下肢功能,并记录术后并发症。结果 42例患者平均获22.64±3.88个月随访,平均年龄73.55±2.42岁。其中,男23例,女19例。术前平均血红蛋白为94.31±15.79g/l,平均手术时长19.40±4.44 min,平均术中出血量15.26±2.88 ml,平均骨折愈合时间9.50±2.23周,末次随访颈干角120.9±7.13°。Harris髋关节评分结果:优23例,良13例,可2例,差1例,优良率92.3%。针道感染4例,因其他疾病死亡3例。结论 外固定架治疗高风险老年患者股骨粗隆间骨折是一种切实可行的手术方案。其手术时间短、手术创伤小,允许术后早期下地活动,骨折复位优良。
Objective To evaluate the outcomes of external fixation in the treatment of intertrochanteric fracture in high-risk elderly patients. Methods From June 2014 to March 2019, the clinical data of patients with intertrochanteric fracture treated by external fixation were retrospectively analyzed. The hemoglobin, operation time, intraoperative blood loss, neck-shaft angle, duration of fracture union and follow-up time were recorded. The Harris hip score was used to evaluate lower extremity function at the last follow-up, and postoperative complications were recorded. Results A total of forty-two patients with intertrochanteric fracture were treated by the external fixation,with a mean follow-up time of 22.64 ± 3.88 months. There were 23 were males and 19 were females with a mean age of 73.55 ± 2.42years. The mean preoperative hemoglobin was 94.31 ± 15.79 g/l, the mean operation time was 19.40 ± 4.44 min, the mean intraoperative blood loss was 15.26 ± 2.88ml, the mean fracture union time was 9.50 ± 2.23 weeks, and the neck-shaft angle was 120.9 ± 7.13 ° at the final follow-up. The results of Harris hip classification: 23 cases of excellent, 13 cases of good, 2 cases of fair, and one case of poor, with an excellent and good rate of 92.3%. There were 4 cases of pin tract infection and 3 patients of death since other diseases. Conclusion External fixation is a practical method for the treatment of intertrochanteric fracture in high-risk elderly patients, with a short operation time, less surgical damage, early postoperative rehabilitation, and satisfactory fracture union.
