摘要
目的:探究老年股骨转子间骨折闭合复位内固定术后内固定失败的影响因素。方法:回顾性分析接受闭合复位内固定术治疗的老年股骨转子间骨折患者90例,其中,动力髋螺钉内固定术(DHS)、创复位术结合加长型抗旋转型股骨近端髓内钉内固定术(PFNA)治疗患者各45例,观察手术固定结果并分析影响固定失败的因素。结果:PFNA术组治疗成功率、失败率、并发症发生率与DHS术组患者相比未见明显区别,差异无统计学意义(P>0.05)。PFNA术组内固定失败原因中骨质疏松、顶尖距>25 cm、不稳定型骨折及骨折功能复位与DHS术组患者相比未见明显区别,差异无统计学意义(P>0.05)。结论:老年股骨转子间骨折闭合复位内固定术后内固定失败的原因主要有骨质疏松、顶尖距>25 cm、不稳定型骨折及骨折功能复位等,需加大该方面的研究力度,提高治疗成功率。
Objective To explore the influencing factors of internal fixation failure after closed reduction and internal fixation of intertrochanteric fractures in the elderly. Method 90 cases of intertrochanteric fractures were treated with closed reduction and internal fixation. Among them,dynamic hip screw fixation（ DHS）,combined with lengthened anti-rotation proximal femur 45 patients were treated with PFNA. The fixation results were analyzed and the factors affecting the failure of fixation were analyzed. Results The success rate,failure rate and complication rate of PFNA group were not significantly different from those of DHS group（ P〉0. 05）. There was no significant difference in osteoporosis,tip distance 25 cm,unstable fracture and functional reduction of fracture in PFNA group compared with DHS group（ P〉0. 05）. Conclusion The causes of failure of internal fixation of intertrochanteric fractures in senile patients with intertrochanteric fractures are osteoporosis,distal distance 〉25 cm,unstable fracture and functional reduction of fracture. Therefore,it is necessary to intensify the research and improve the treatment Success rate.
