摘要
目的了解中重度老年痴呆症患者家庭抗逆力现状,并分析其影响因素。方法采用便利抽样法,于2021年4—12月纳入北京市某三级甲等医院神经内科门诊130例中重度老年痴呆症患者家庭,采用家庭抗逆力量表、家庭一致感量表、照顾积极感受量表、Zarit照顾者负担量表、家庭管理资源量表、家庭危机个人评价量表、家庭亲密度与适应性量表和领悟社会支持量表进行调查,并对数据进行分析。结果130例中重度老年痴呆症患者家庭抗逆力得分为(58.79±5.78)分,患者出现症状至确诊间隔时长、家庭世界观、家庭解决问题与应对方式、家庭资源是中重度老年痴呆症患者家庭抗逆力的主要影响因素(均P<0.05),共同解释家庭抗逆力得分总变异的32.8%。结论北京市某三级甲等医院中重度老年痴呆症患者家庭抗逆力处于中等水平,老年痴呆症患者疾病的早期识别与诊断、正向的家庭世界观和积极的解决问题与应对方式、可充分获取和利用的家庭资源均有助于提升中重度老年痴呆症患者的家庭抗逆力。
Objective To investigate the current status of family resilience in patients with moderate-severe dementia,analyze the influencing factors.Methods The families of 130 patients with moderate-severe dementia were collected from neurology clinic of a grade-A tertiary hospital in Beijing from April to December 2021 by the convenience sampling method.The investigation was performed by the family hardiness index scale,family sense of coherence scale,positive aspects of caregiving scale,Zarit caregiver burden scale,family inventory of resources for management scale,family crisis-oriented personal evaluation scales,family intimacy and adaptability scale,and perceived social support scale,and the data were analyzed.Results The score of family resilience in families of 130 patients with moderate-severe dementia was(58.79±5.78)points.The influencing factors of family resilience of patients with moderate-severe dementia included duration between symptom onset and diagnosis,family coherence,family problem solving and coping styles,family resources,jointly explaining 32.8%of the total variation of family stress resistance score.Conclusion The family resilience of patients with moderate-severe dementia of a grade-A tertiary hospital in Beijing is at medium level,and the early identification and diagnosis of dementia,positive family coherence and positive problem-solving and coping styles,and family resources that can be fully obtained and utilized can help to improve the family resilience of patients with moderate-severe dementia.
