不同时期行颅骨缺损修补术对颅脑外伤去骨瓣减压术后伴脑积水患者颅压及神经功能的影响 被引量:7

摘要 目的 探讨不同时期行颅骨缺损修补术对颅脑外伤去骨瓣减压术后伴脑积水患者颅压及神经功能的影响。方法 将我院2018年7月至2019年7月收治的125例颅脑外伤去骨瓣减压术后伴脑积水患者按照手术方式分组,对照组62例侧脑室-腹腔(V-P)分流术后3~6个月行颅骨缺损修补术,观察组63例V-P分流术后同期行颅骨缺损修补术。观察两组患者脑积水分流效果、颅压水平、神经功能变化及术后并发症发生情况。结果 两组脑积水分流效果对比差异无统计学意义(P>0.05);观察组术后颅内压较对照组低(P<0.05);观察组术后GOS评分较对照组高,NIHSS评分较对照组低(P<0.05);观察组术后并发症发生率(4.76%)较对照组(16.13%)低(P<0.05)。结论 V-P分流术后同期予以颅脑修补术可通过维持颅脑外伤去骨瓣减压术后伴脑积水患者颅内空间平衡,促进脑积水分流,维持颅内压水平、改善神经功能及降低术后并发症。 Objective To investigate the effect of skull defect repair in different periods on the intracranial pressure and nerve function of patients with hydrocephalus after craniocerebral trauma after decompressive craniectomy.Methods 125 cases of craniocerebral trauma patients with hydrocephalus craniocerebral trauma after decompressive craniectomy from July 2018 to July 2019 were divided into groups according to the surgical method, and 62 cases of patients give Skull defect repair.3~6 months after lateral ventricle-abdominal(V-P)shunt, and 63 cases in the observation group were treated with skull defect repair at the same time after VP shunt.The effects of hydrocephalus flow, intracranial pressure levels, changes in nerve function and postoperative complications were observed in the two groups.Results There was no significant difference in the effect of hydrocephalus flow between the two groups(P>0.05);the postoperative intracranial pressure of the observation group was lower than the control group(P<0.05);the postoperative GOS score of the observation group was higher than the control group, and the NIHSS score was lower than the control group(P<0.05);the postoperative complication rate of the observation group(4.76%)was lower than the control group(16.13%)(P<0.05).Conclusion Craniocerebral repair at the same time after VP shunt can maintain the balance of intracranial space in patients with hydrocephalus craniocerebral trauma after decompressive craniectomy, promote hydrocephalus flow, maintain intracranial pressure, improve nerve function and reduce postoperative complications.
