聚醚醚酮与钛网在去骨瓣减压术后颅骨修补中的临床效果比较 被引量:1

摘要 目的通过比较去骨瓣减压术后聚醚醚酮(PEEK)和钛网进行颅骨修补术后的临床应用效果,为两种材料的选择提供指导意见。方法回顾性分析河南中医药大学第一附属医院神经外科自2015年4月至2021年4月收治的所有去骨瓣减压术后行颅骨修补术患者的临床资料。对患者的人口学特征、并发症和术后疗效进行总结分析。结果共纳入133例行颅骨缺损修补术的患者:钛网组97例(72.9%);PEEK组36例(27.1%)。钛网组和PEEK组术后总的并发症发生率相似(29.9%比31.7%)(P>0.05);两组术后颅内出血、癫痫、感染、硬膜下积液、二次手术发生率以及手术时间、术中出血量、住院时间比较,差异无统计学意义(P>0.05);PEEK组皮下积液发生率及医疗总费用均高于钛网组(P<0.05);两组患者对颅骨修补术后的满意度均较高,但PEEK组高于钛网组(P<0.05)。结论钛网和PEEK用于颅骨修补术的手术结果相似,但需要注意PEEK修补患者术后皮下积液并发症的处理。钛网具有更高的性价比,但是有一定经济基础的患者可选择PEEK材料。 Objective To compare the clinical effect of polyetheretherketone(PEEK) and titanium mesh in cranioplasty after decompressive craniectomy to provide guidance for the selection of two materials.Methods The clinical data of all patients admitted to the First Affiliated Hospital of Henan University of Chinese Medicine from April 2015 to April 2021 who undwent cranioplasty after decompressive craniotomy were retrospectively analyzed. Patient demographics characteristics, complications and postoperative efficacy of the patients were abstracted and analyzed.Results A total of 133 patients who had underwent cranioplasty were included 97 patients(72.9%) in the titanium mesh group and 36 patients in the PEEK group(27.1%). Overall complication rates were similar between the titanium mesh group(29.9%) and PEEK group(31.7%)(P>0.05). There were no differences in postoperative bleeding, epilepsy, infection, subdural effusion, incidence of secondary surgery, operative time, intraoperative blood loss and length of stay(P>0.05). The incidence of subcutaneous effusion and surgical cost in PEEK group were higher than those in titanium mesh group(P<0.05). All patients showed satisfaction with cranioplasty, but PEEK group was higher than titanium mesh group(P<0.05).Conclusion Titanium mesh and PEEK surgical results for cranioplasty are similar but it is necessary to pay attention to the complications of subcutaneous hydrops after PEEK patients. Titanium mesh has higher performance cost ratio, but patients with a certain economic basis can choose PEEK material.
