摘要
患者是临床诊疗的关键环节,以患者为中心的诊疗模式备受国际关注。“患者价值观与偏好”研究是该模式在临床实践指南制定过程中的体现,可充分体现“以人为本”的核心思想。中医的受众人群文化背景不同,决定了中医临床实践指南中患者价值观与偏好不能完全套用目前既有领域的研究,需要识别其中重点影响因素并针对问题提出解决方案。认为中医临床实践指南研究过程应重视“患者价值观与偏好”,从中医理论及临床实践的自身特点、要素出发,丰富已有研究体系的维度构建。丰富维度构建时出现的各种风险,可通过多群体横断面调研后的抽样调查、共享决策、焦点小组访谈、医疗最大最小化量表等方法探寻解决方案,从而更好地收集、整合、汇报患者价值观与偏好,为中医临床实践指南制定时患者价值观与偏好部分的编写提供思路。
The mode of patient-centered care(PCC)has attracted global attention as patients are the center of clinical practice.The study on patient values and preferences is the embodiment of PCC mode in the development of clinical practice guidelines,fully reflecting the people-oriented idea.Considering the varied cultural background of the recipients of traditional Chinese medicine(TCM),the general research approach of patient values and preference cannot be directly used in developing TCM clinical practice guidelines,but should be modified regarding the key influence factors.To put focus on the patient values and preferences in TCM clinical practice guidelines,it is suggested to enrich the dimensions of the existing research system from the characteristics and key elements of TCM theory and clinical practice.Regarding the risks in constructing the enriching the dimensions,the solutions are provided including sampling survey after multi-group cross-sectional survey,shared decision making,focus group interview,medical maximizaing-minimizing scale.In this way,patient values and preferences can be better collected,integrated and reported,providing references for the compilation of patient values and preferences in the development of TCM clinical practice guidelines.
