摘要
目的:分析冲击波联合康复手法对外伤后膝关节功能障碍患者疼痛、关节活动度和膝关节功能的影响。方法:选取符合纳入标准的65例外伤后膝关节功能障碍患者,依据随机数字表随机分为对照组(32例)和联合组(33例)。对照组采用康复手法(推拿+松动手法)进行治疗,联合组在对照组治疗方案的基础上联合放射状冲击波治疗。比较两组治疗前和治疗8周后视觉模拟评分(VAS)、膝关节屈伸角度和Lysholm膝关节功能评分的变化情况。结果:治疗8周后,两组VAS评分均较治疗前明显降低(P <0.01),且联合组VAS评分较对照组明显降低(P <0.01)。相比治疗前,两组治疗8周后膝关节屈伸角度均明显提高(P <0.01),且联合组治疗后膝关节屈伸角度较对照组明显提高(P <0.01)。两组治疗8周后,Lysholm膝关节功能评分较治疗前均显著提高(P <0.01),且联合组治疗后Lysholm评分较对照组显著提高(P <0.01)。结论:放射状冲击波联合康复手法可有效减轻外伤后膝关节功能障碍患者局部疼痛,对关节功能具有良好的改善作用,治疗效果肯定。
Objective:To analyze the effects of shock wave combined with rehabilitation manipulation on pain,range of motion and function of knee joint in patients with post-traumatic knee joint dysfunction.Methods:65 patients with post-traumatic knee joint dysfunction met the inclusion criteria were randomly divided into the control group(n = 32)and the combined group(n = 33).The control group was treated with rehabilitation manipulation(massage + loosening manipulation);on which basis,the combined group was also treated with radial shock wave therapy.The changes of VAS,the angle of knee flexion and extension,as well as the scores of Lysholm knee function were compared between before and after 8 weeks of the treatment in the two groups.Results:After 8 weeks of the treatment,the VAS scores were significantly decreased in the two groups(P < 0.01),and the decreases were more significant in the combined group than those in the control group(P < 0.01).The angles of knee flexion and extension were significantly increased after 8 weeks of the treatment compared with those before the treatment in the two groups(P < 0.01),and the increases were more significant in the combined group than those in the control group(P < 0.01).The scores of Lysholm knee function were significantly higher after 8 weeks of the treatment than those before the treatment in the two groups(P < 0.01),and the scores were much higher in the combined group than those in the control group after the treatment(P < 0.01).Conclusion:Radial shock wave combined with rehabilitation manipulation can effectively relieve local pain of knee joint dysfunction and improve motor function of knee joint in the patients with post-trauma.
