摘要
目的:观察季节变化对抑郁症患者褪黑素(MT)与HPA轴激素的影响,以探讨“肝应春”的生物学基础。方法:收集抑郁症组患者67例和健康组40例纳入研究。于春分、夏至、秋分、冬至进行样本采集,比较组间促肾上腺皮质激素释放激素(CRH)、促肾上腺皮质激素(ACTH)以及皮质醇(CORT)的水平。采用Pearson相关性分析研究不同季节之间以及某一季节同组内的MT与HPA轴激素的相关性。结果:健康组中,HPA轴激素CRH、ACTH、CORT四季分泌趋势均为夏季<春季<秋季<冬季。抑郁组中,CRH抑郁组四季趋势为春季<夏季<冬季<秋季,ACTH与CORT抑郁组四季分泌趋势均为夏季<春季<冬季<秋季。与健康组比较,抑郁症组HPA轴激素整体显著升高(P<0.05,P<0.01)。四季MT与HPA轴激素季节性变化的相关性结果显示,四季健康组MT与CRH、ACTH、CORT呈强正相关(P<0.05),春季抑郁症组MT与CRH、ACTH、CORT呈强负相关(P<0.05);冬季健康组MT与CRH、ACTH、CORT呈强正相关(P<0.05)。结论:MT是季节变化对HPA轴激素的调节中介之一,其调节机制具有季节选择性和复杂性。MT介导的HPA轴激素的季节性变化可能是中医“肝应春,主疏泄、调节情志”的科学内涵之一。
Objective:To observe the influence of seasonal changes on melatonin(MT)and HPA axis hormones and to explore the biological basis of‘liver response to spring’.Methods:A total of 67 patients in the depression group and 40 volunteers in the normal group were enrolled in the study.Samples were collected at the vernal equinox,autumnal equinox,summer solstice,and winter solstice to compare the levels of corticotropin-releasing hormone(CRH),corticotropin(ACTH)and cortisol(CORT)between groups.Pearson correlation analysis was used to study the correlation between MT and HPA axis hormones between different seasons and within the same group in a certain season.Results:In the normal group,the secretion trends of HPA axis hormones CRH,ACTH and CORT in all seasons were summer<spring<autumn<winter.In the depression group,the CRH depression group’s four seasons trends were spring<summer<winter<autumn,and the ACTH and CORT depression groups’four seasons secretion trends were summer<spring<winter<autumn.The data between groups showed that compared with the normal group,the overall HPA axis hormones in the depression group were significantly increased(P<0.05,P<0.01).The correlation between MT in four seasons and seasonal changes of HPA axis hormones in the group showed that MT in the four seasons normal group was strongly positively correlated with CRH,ACTH and CORT(P<0.05),MT in the spring depression group was strongly negatively correlated with CRH,ACTH and CORT(P<0.05);MT in the winter normal group was significantly positively correlated with CRH,ACTH and CORT(P<0.05).Conclusion:MT is one of the mediators of seasonal changes on HPA axis hormones,and its regulatory mechanism is seasonally selective and complex.The seasonal change of HPA axis hormone mediated by MT may be one of the scientific connotations of‘liver correspond with spring,governs dredging and regulating and emotion’in traditional Chinese medicine.
