摘要
目的 通过上海局部地区对702例冠心病病史资料进行回顾性分析,研究冠心病发病的时间分布,以阐明疾病中医时间学规律,为疾病的临床诊疗及建立有效的应急控制体系提供理论参考。方法 收集符合中西医诊疗标准的2011年—2015年(农历辛卯年至乙未年)上海中医药大学附属曙光医院东院住院病人病史资料,并进行回顾性分析。采用SPSS 21.0软件进行统计检验,圆分布统计法计算各证型发病集中趋势。结果 ①纳入冠心病病例共702例,其中男558例(79.49%),女144例(20.51%);年龄20~99岁。②圆分布统计结果认为病人发病时间不存在集中趋势,雷达图示在冬末春初时段及夏季时段发病人数较多,在立春、立夏、立秋、立冬节气中也有高发病趋势。③不同年份冠心病病人由高到低依次为:甲午年(172例)>乙未年(154例)>壬辰年(147例)>癸巳年(121例)>辛卯年(108例)。④冠心病不同证型及证素病人发病情况不同。结论 上海局部地区冠心病发病人数存在节气、季节等时间差异,时间节律对冠心病发病有一定的预测意义。
Objective To analyse the attack time distribution of coronary heart disease(CHD)based on the retrospective analysis of the medical records of 702 patients with CHD.Methods The medical records of inpatients in the east branch,Shuguang Hospital,Shanghai University of Traditional Chinese Medicine from 2011 to 2015 were collected and analyzed retrospectively.SPSS 21.0 software was used for statistical testing,and circular distribution statistical method was used to calculate the central trend of each syndrome type.Results①A total of 702 CHD cases were included,including 558 males(79.49%)and 144 females(20.51%).The ages were from 20 to 99 years old.②There was no central trend in the onset time of attack of CHD.The radar map showed that the onset time was concentrated in the late winter,early spring,and summer,and there was a high incidence trend in Spring Begins,Summer Begins,Autumn Begins,and Winter Begins.③The frequency of patients with CHD was:Jiawu year(172)>Yiwei year(154)>Renchen year(147)>Guisi year(121)>Xinmao year(108).④The incidence of CHD was different in syndrome types and syndrome elements.Conclusion The onset of CHD in local areas of Shanghai is different in solar terms and seasons.Time rhythm could supply certain clne in predicting the attack of CHD.
