FSHR基因多态性与卵巢反应性及临床结局的相关研究 被引量:2

Association of FSHR gene polymorphisms with ovarian responsiveness and clinical outcome

摘要 目的探讨卵泡刺激素受体(FSHR)基因多态性与卵巢反应性及妊娠结局的关系。方法招募2020年10月至2021年12月在中国人民解放军总医院第一医学中心生殖医学中心行IVF-ET助孕治疗的551例不孕症女性为研究对象。根据FSHR多态性位点基因型检测结果,患者基于rs6165(Thr307Ala)分为A/A组(273例)、A/G组(224例)和G/G组(54例);且基于rs6166(Asn680Ser)亦分为A/A组(272例)、A/G组(231例)和G/G组(48例)。比较各组患者的一般资料、促排卵及胚胎发育情况、卵巢反应情况、临床妊娠率及卵巢过度刺激综合征(OHSS)发生率。结果纳入的551例助孕女性中,FSHR单核苷酸多态性(SNP)位点rs6165(Thr307Ala)A/A基因型和rs6166(Asn680Ser)A/A基因型是最常见的基因型,频率分别为49.54%和49.36%。rs6165(Thr307Ala)A/A基因型组的MⅡ卵母细胞数、正常受精数显著高于A/G基因型组(P<0.05),卵巢低反应发生率显著低于A/G和G/G基因型组(P<0.05)。rs6166(Asn680Ser)A/A基因型组的MⅡ卵母细胞数、正常受精数和正常卵裂数显著高于A/G基因型组(P<0.05),卵巢低反应发生率亦显著低于A/G和G/G基因型组(P<0.05)。rs6165和rs6166各基因型组的临床妊娠率及OHSS发生率均无显著性差异(P>0.05)。结论FSHR常见SNP位点rs6165(Thr307Ala)和rs6166(Asn680Ser)的不同基因型与卵巢反应性有一定关联,可以为临床制定促排卵方案提供参考;但其对妊娠结局及OHSS发生的影响并不明显。 Objective:To investigate the correlation between FSH receptor(FSHR)gene polymorphisms and ovarian responsiveness and pregnancy outcome.Methods:A total of 551 women who underwent IVF-ET assisted pregnancy treatment in the Assisted Reproductive Medicine Center of the Chinese PLA General Hospital from October 2020 to December 2021 were recruited as the research objects.According to the genotypes of FSHR polymorphic loci,they were divided into A/A group(n=273),A/G group(n=224)and G/G group(n=54)based on rs6165(Thr307ala),and they were also divided into A/A group(n=272),A/G group(n=231)and G/G group(n=48)based on rs6166(Asn680Ser).The general data,ovulation induction and embryonic development,ovarian response,clinical pregnancy rate and the incidence of ovarian hyperstimulation syndrome(OHSS)were compared among the groups.Results:Among 551 pregnant women,the single nucleotide polymorphism(SNP)loci of FSHR i.e.rs6165(Thr307Ala)A/A genotype and rs6166(Asn680Ser)A/A genotype were the most common genotypes,with frequencies of 49.54%and 49.36%respectively.The number of MⅡoocytes and normal fertilization in rs6165(Thr307 Ala)A/A genotype group were significantly higher than that in A/G genotype group(P<0.05),and the incidence of OHSS was significantly lower than that in A/G and G/G genotype groups(P<0.05).The number of MⅡoocytes,normal fertilization and normal cleavage in rs6166(asn680Ser)A/A genotype group were significantly higher than those in A/G genotype group(P<0.05),and the incidence of OHSS was also significantly lower than that in A/G and G/G genotype groups(P<0.05).There was no significant difference in clinical pregnancy rate and OHSS incidence between rs6165 and rs6166 genotype groups(P>0.05).Conclusions:Common SNPs sites of FSHR i.e.rs6165(Thr307Ala)and rs6166(Asn680Ser),were associated with ovarian responsiveness,which can provide a reference for the clinical formulation of ovulation induction protocol.However,its effect on pregnancy outcome and OHSS is not obvious.
