摘要
目的探讨经导管肝动脉化疗栓塞(TACE)序贯微波消融术(MWA)联合阿帕替尼治疗不可切除肝癌的疗效及安全性。方法选取2018年1月至2020年1月收治的40例中晚期肝癌患者,均无外科手术切除指征,采用TACE序贯MWA联合阿帕替尼治疗的方案。根据改良版实体瘤疗效评价标准(mRECIST)评估肿瘤反应,分析TACE序贯MWA联合阿帕替尼治疗的患者临床疗效,同时观察其不良反应情况。结果本组患者术后3个月、6个月、12个月的客观缓解率和疾病控制率分别为62.5%、65.0%、52.5%和97.5%、85.0%、78.9%。患者中位生存时间28个月,6个月、1年、2年累积生存率分别为95.0%、67.5%、37.5%。大多数患者不良反应轻微,可耐受。结论TACE序贯MWA联合阿帕替尼治疗不可切除肝癌疗效确切,且安全性良好。
Objective To investigate the efficacy and safety of sequential microwave ablation(MWA)combined with Apatinib in the treatment of unresectable hepatocellular carcinoma(HCC).Methods A total of 40 patients with intermediate and advanced liver cancer admitted from January 2018 to January 2020 without indications for surgical resection were selected and treated with TACE sequential MWA combined with Apatinib.The tumor response was evaluated according to the modified Solid Tumor Efficacy Evaluation Criteria(mRECIST),and the clinical efficacy of TACE sequential MWA combined with Apatinib was analyzed,and the adverse reactions were observed.Results The objective remission rate and disease control rate were 62.5%,65.0%,52.5%and 97.5%,85.0%,78.9%at 3,6 and 12 months after operation.The median survival time was 28 months,and the cumulative survival rates at 6 months,1 year and 2 years were 95.0%,67.5%and 37.5%,respectively.Adverse reactions were mild and tolerable in most patients.Conclusion TACE sequential MWA combined with Apatinib is effective and safe in the treatment of unresectable liver cancer.
