IDEAL-IQ成像在肝移植术后肝脏早期脂肪变性观察的应用

摘要 目的:应用磁共振IDEAL-IQ成像技术对肝移植术后患者早期肝脏脂肪变性的变化进行随访观察。方法:回顾性分析2018年8月-2021年1月在我院101例行同种异体肝移植手术患者的影像资料和术后肝活检穿刺结果。根据术后影像检查时间分三组:分别是术后3月内组(3M组)、术后3~6月组(3~6M组)、术后6~12月组(6~12M组)。结果:脂肪比值分别是:2.72±0.755(P=0.20),2.45(2-9)(P=0.021),6.32(2-15)(P<0.001);3M组的脂肪比值与3~6M组总体分布相同(P=0.18),而3M组与6~12M组以及3~6M组与6~12M组的脂肪比值总体分布不同(P=0.02,P=0.04)。三组组织学检查移植肝脂肪变性发生率分别是:14.3%,15.8%,23.5%;3M′组的肝细胞脂肪变性发生率与3~6M′组无统计学差异(P=0.431),而3M′组与6~12M′组以及3~6M′组与6~12M′组的肝细胞脂肪变性发生率不同(P=0.028,P=0.040)。ROC曲线评价IDEAL-IQ诊断移植肝脂肪变性诊断效能:以界值4.45诊断脂肪变性的敏感度为0.823,特异度为0.859,ROC曲线下面积为0.907(P<0.01)。结论:移植肝术后1年内会发生肝细胞脂肪变性,于移植术6个月后出现肝细胞脂肪变性比率提高,应用磁共振IDEAL-IQ脂肪定量测量技术对诊断移植肝脂肪变性具有较高的敏感性和特异性,可以作为肝移植术后对患者肝细胞脂肪变性长期随访观察的一种检查技术。 Objective:Follow-up observation of the changes of early liver steatosis in patients after liver transplantation with MRI IDEAL-IQ imaging technology.Methods:We Retrospective analysis of imaging data and postoperative liver biopsy results of 101 patients after liver transplantation in our hospital from August 2018 to January 2021. divided into three groups according to the time of postoperative imaging examination :postoperative within 3 months group(3 M group), postoperative in 3~6 months group(3~6 M group), postoperative in 6~12 months group(6~12 M group),respectively.Results:;Fat ratio were 2.72±0.755(P=0.20), 2.45(2-9)(P=0.021),6.32(2-15)(P<0.001),respectively;The overall distribution of fat ratio in the 3 M group was the same as that in the 3~6 M group(P=0.18), while the overall distribution of fat ratio in the 3 M group was different from the 6~12 M group, and the 3~6 M group was also different from the 6~12 M group(P=0.02, P=0.04).The incidence of hepatic steatosis in the three groups of histological examination were14.3%, 15.8% and 23.5%, respectively. The incidence of hepatocyte steatosis in the 3 M′ group was the same as that in the 3~6 M′ group(P=0.431), while the incidence of hepatocyte steatosis was different from 3 M′ group and 6~12 M′ group, and also different from 3~6 M′ group and 6~12 M′ group(P=0.028, P=0.040). ROC curve evaluated the diagnostic efficiency of IDEAL-IQ in diagnosing steatosis of transplanted liver: the sensitivity of diagnosing steatosis with a cut-off value of 4.45 was 0.823, the specificity was 0.859, and the area under the ROC curve was 0.907(P<0.01).Conclusion:Hepatocyte steatosis may occur within 1 year after liver transplantation, and progress may occur after 6 months transplantation.application of magnetic resonance(NMR) IDEAL-IQ fat quantitative measurement technology to diagnose hepatic steatosis has high sensitivity and specificity, can serve as after liver transplantation for patients with hepatocellular fatty degeneration and long-term follow-up of a inspe
