摘要
腰麻后低血压(PSH)是剖宫产术中常见的并发症,主要是由交感神经阻滞引起的周围血管阻力降低和妊娠子宫压迫下腔静脉使回心血量减少所致。PSH不但会使产妇发生一系列不良反应,而且会使子宫胎盘血流量下降,导致胎儿功能性缺氧和酸中毒,严重威胁着产妇及婴儿的安全。该文对近年来剖宫产腰麻后低血压的预测、防治措施等方面的临床研究进行综述,旨在加强剖宫产围术期管理,降低产妇PSH的风险,改善母婴的预后,以期为临床应用和研究提供参考。
Post-spinal hypotension(PSH) is a common complication of cesarean section.It is mainly caused by the reduction of peripheral vascular resistance caused by sympathetic nerve block and the reduction of blood return to the heart caused by the compression of the inferior vena cava by the pregnant uterus.PSH will not only cause a series of adverse reactions in the maternity,but also reduce the uteroplacental blood flow,resulting in functional hypoxia and acidosis of the fetus,which seriously threatens the safety of maternity and babies.This article reviews the clinical research on the prediction,prevention and treatment of hypotension after cesarean section and spinal anesthesia in recent years.It aims to strengthen the perioperative management of cesarean section,reduce the risk of maternal PSH,and improve the prognosis of mother and baby,in order to provide reference for clinical application and research.
