摘要
目的探讨香菇多糖联合奥沙利铂+亚叶酸钙+氟尿嘧啶(FOLFOX6)治疗对晚期胃肠道恶性肿瘤患者淋巴细胞亚型、系统免疫炎症指数(systemic immune-inflammation index,SII)的影响。方法选择作者医院晚期胃肠道恶性肿瘤患者70例,按照入院先后顺序分为对照组和观察组,每组35例,两组均进行FOLFOX6治疗,观察组在此基础上给予香菇多糖,21天为1个周期,均治疗4个周期。比较两组临床疗效,治疗前后炎症因子、淋巴细胞亚型、SII、卡氏功能状态评分(Karnofsly′s performance status,KPS)、生活质量以及不良反应。结果观察组客观缓解率(objectivere-spon serate,ORR)、疾病控制率(diseasecon trolrate,DCR)与对照组比较差异无统计学意义(P>0.05)。治疗后,两组白细胞介素6(interleukin-6,IL-6)、IL-8、CD8^(+)水平及SII均降低(P均<0.05),CD3^(+)、CD4^(+)水平及KPS均升高(P均<0.05),且观察组IL-6、IL-8、CD8^(+)水平及SII均低于对照组(P均<0.05),CD3^(+)、CD4^(+)水平及KPS均高于对照组(P均<0.05)。治疗后,观察组生活质量各指标评分均优于对照组(P<0.05)。两组恶心/呕吐、贫血、肝损害发生率差异均无统计学意义(P均>0.05),观察组白细胞减少、骨髓抑制、粒细胞减少和血小板减少发生率均低于对照组(P均<0.05)。结论香菇多糖联合FOLFOX6治疗可有效改善晚期胃肠道恶性肿瘤患者淋巴细胞亚型、SII,提高免疫功能和生活质量,且安全性好。
Objective To investigate the effects of lentinus polysaccharide combined with FOLFOX6 treatment on lymphocyte subtype and systemic immune-inflammation index(SII)in patients with advanced gastrointestinal malignancy.Methods A total of 70 patients with advanced gastrointestinal malignancy in author′s hospital were selected and divided into control group(n=35)and observation group(n=35)according to the order of admission with both groups underwent FOLFOX6 treatments,in addition,the observation group was given lentinus polysaccharide on these basis,with 1 cycle of 21 days,all treated for 4 cycles.The clinical efficacy of the two groups and inflammatory factors,lymphocyte subtype,SII,Karnofsly′s performance status(KPS)score,quality of life,adverse effects before and after treatments were campared.Results There was no significant difference in objective response rate(ORR),disease control rate(DCR)between the observation group and the control group(P>0.05).After treatments,The levels of interleukin-6(IL-6),IL-8,CD8^(+)and SII were decreased(all P<0.05),the levels of CD3^(+),CD4^(+)and KPS were increased(all P<0.05),and the levels of IL-6,IL-8,CD8^(+)and SII in observation group were lower than control group(all P<0.05),while the levels of CD3^(+),CD4^(+)and KPS score were higher than control group(all P<0.05).After treatments,the scores of life quality in the observation group were better than those in the control group(P<0.05).There was no significant difference in the incidence of nausea/vomiting,anaemia and liver damage between the two groups(all P>0.05),and the incidence of leukocytopia,myelosuppression,granulocytopenia and thrombocytopenia in observation group were lower than the control group(all P<0.05).Conclusion Mushroom polysaccharide combined with FOLFOX6 treatment can effectively improve lymphocyte subtype and SII,and improve immune function and quality of life in patients with advanced gastrointestinal malignancy,which is more safety.
