摘要
目的系统评价早期康复运动对急性心肌梗死(acute mycardia infarctin,AMI)患者经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)术后疗效的影响。方法计算机检索维普、万方、CNKI、SinoMed、PubMed、Web of Science、Embase、Cochrane Library等多个中英文数据库中有关早期康复运动对PCI术后患者疗效影响的随机对照试验(randomized controlled trial,RCT),检索时间为建库至2021年8月,使用Revman 5.4软件进行Meta分析。结果共纳入16项研究,结果显示,与对照组相比,试验组术后左心室射血分数(left ventricular ejection fraction,LVEF)增加(MD=5.18,95%CI:4.13~6.22,P<0.001);6 min步行距离(6 minutes walking distance,6MWD)延长(MD=58.65,95%CI:24.12~93.19,P<0.001);住院时间缩短(MD=−2.11,95%CI:-2.26~−1.97,P<0.001);术后并发症发生率降低(OR=0.17,95%CI:0.10~0.27,P<0.001)。结论早期康复运动可以促进AMI患者PCI术后心功能的恢复,缩短患者住院时间,降低术后并发症发生率。
Objective:To systematically evaluate the effect of early rehabilitation exercise on patients with acute myocardial infarction after percutaneous coronary intervention.Methods:The randomized controlled trials on the impact of early exercise rehabilitation on the quality of life of patients after PCI in several Chinese and English databases such as VIP,Wanfang Data,CNKI,SinoMed,PubMed,Web Of Science,EMbase and Cochrane Library were searched by computer.The retrieval time was up to August 2021,and revman 5.4 software was used for Meta-analysis.Results:A total of 14 literatures were included.The results of Meta-analysis showed that compared with the control group,the left ventricular ejection fraction in the experimental group increased(MD=5.18,95%CI:4.13~6.22,P<0.001);The 6 minute walking distance was prolonged(MD=58.65,95%CI:24.12~93.19,P<0.001);The length of hospital stay was shortened(MD=−2.11,95%CI:−2.26~−1.97,P<0.001);The incidence of postoperative complications decreased(OR=0.17,95%CI:0.10~0.27,P<0.001).Conclusion:Current evidence shows that early rehabilitation exercise can promote the recovery of cardiac function,shorten hospital stay and reduce the incidence of postoperative complications in patients with acute myocardial infarction after PCI;The above conclusions still need to be verified by high-quality research so as to provide more evidence-based basis for early rehabilitation exercise after PCI.
