摘要
目的探讨急性髓系白血病(acute myeloid leukemia,AML)患者外周血CD8^(+)T淋巴细胞中Mg^(2+)转运蛋白1(MagT1)水平监测的临床价值。方法收集2014年3月至2018年12月收治的新发AML患者197例,同期体检者60例。分离外周血单个核细胞(PBMCs)和CD8^(+)T细胞备用。全自动生化分析仪检测血清Mg^(2+)浓度,RT-qPCR检测PBMCs中MagT1 mRNA表达。根据NCCN AML指南将AML患者分为低、中、高风险组,RT-qPCR和Western blot检测患者CD8^(+)T细胞中MagT1 mRNA和蛋白表达水平;流式细胞术检测细胞PD-1和Tim-3表达水平;Spearson相关性分析MagT1蛋白与PD-1和Tim-3表达的相关性;Kaplan-Meier曲线分析CD8^(+)T细胞MagT1蛋白表达与患者无事件生存期(EFS)和总生存期(OS)的关系;采用受试者工作特征(ROC)曲线评估MagT1蛋白表达水平对AML患者风险分层及预后的预测价值。结果与对照组比较,AML患者外周血中Mg;浓度降低,PBMCs中MagT1 mRNA相对表达量下调。与低风险组比较,中、高风险组患者CD8^(+)T细胞中MagT1 mRNA和蛋白相对表达量降低,而PD-1和Tim-3表达水平升高。CD8^(+)T细胞中MagT1蛋白与PD-1和Tim-3表达水平呈负相关;MagT1蛋白高表达患者的EFS和OS高于MagT1蛋白低表达患者;MagT1表达水平对AML患者预后(AUC=0.866,95%CI:0.817~0.915)及风险分层(AUC=0.724,95%CI:0.642~0.806)均具有一定的评估价值。结论AML患者外周血CD8^(+)T细胞中MagT1的表达降低与患者不良风险分层、CD8^(+)T淋巴细胞耗竭和不利的生存状况相关,对评估AML患者风险及预测预后有重要价值。
Objective To explore the clinical value of Mg^(2+) transporter 1(MagT1)expression in peripheral blood CD8^(+)T lymphocytes in patients with acute myeloid leukemia(AML).Methods One hundred and nightyseven new-onset AML patients admitted to our hospital from March 2014 to December 2018 and 60 physical examiners in the same period were enrolled in the study.Peripheral blood monocytes cells(PBMCs)and CD8^(+)T cells were isolated.The concentration of serum Mg^(2+) was detected by automatic biochemical analyzer,and the expression of MagT1 mRNA in PBMCs was detected by RT-qPCR.AML patients were divided into low,medium and high risk groups according to NCCN AML guidelines.The expression of MagT1 mRNA and protein in CD8^(+)T cells were detected by RT-qPCR and Western blot.The expression of PD-1 and Tim-3 was detected by flow cytometry.The correlation between MagT1 protein and the expression of PD-1 and Tim-3 was analyzed by Spearson correlation assay.The relationship between MagT1 protein expression in CD8^(+)T cells and event free survival(EFS)and overall survival(OS)were analyzed by Kaplan-Meier curve.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of MagT1 protein expression level in risk stratification and prognosis of AML patients.Results Compared with the control group,the Mg^(2+) concentration in peripheral blood of AML patients decreased,and the relative expression of MagT1 mRNA in PBMCs decreased.Compared with the low-risk group,the relative expression of MagT1 mRNA and protein in CD8^(+)T cells in the medium and high-risk groups decreased,while the levels of PD-1 and Tim-3 increased.The expression of MagT1 protein in CD8^(+)T cells was negatively correlated with the expression levels of PD-1 and Tim-3.The EFS and OS of patients with high expression of MagT1 protein were higher than those with low expression of MagT1 protein.The expression level of MagT1 has evaluation value for prognosis(AUC=0.866,95%CI:0.817~0.915)and risk stratification(AUC=0.724,95%CI:0.642~0.806)of AML p
