摘要
目的探讨人血清25羟维生素D[25(OH)D]与2型糖尿病周围血管病变的相关性。方法回顾性分析2018年12月至2021年1月江西省景德镇市第三人民医院收治的200例2型糖尿病患者的临床资料,将其作为研究组,根据患者的肢体多普勒踝肱指数(ABI)进行分组,将ABI≤0.9者纳入病变组(n=100),将ABI>0.9者纳入未病变组(n=100);选择同期体检中心健康体检者65名作为对照组。比较3组的临床资料和生化指标[糖化血红蛋白(HbAlc)、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、同型半胱氨酸(Hcy)、血清尿酸(SUA)、25(OH)D],并给予Pearson相关分析25(OH)D与2型糖尿病周围血管病变的相关性。结果病变组的吸烟率、年龄、体质量指数高于未病变组和对照组,病变组的糖尿病病程长于未病变组,未病变组的高血压患病率高于对照组(P<0.05);未病变组与对照组的吸烟率、体质量指数比较,差异无统计学意义(P>0.05)。未病变组与病变组的HbAlc、FPG、TC高于对照组,25(OH)D、HDL-C低于对照组(P<0.05);病变组的25(OH)D、HDL-C低于未病变组,SUA高于未病变组(P<0.05)。病变组中25(OH)D水平<20.0 ng/ml比例高于未病变组(P<0.05)。Pearson相关分析显示,血清25(OH)D与HbAlc、FPG、TC、SUA呈负相关。结论血清25(OH)D下降会增加2型糖尿病周围血管病变的风险,该指标与HbAlc、FPG、TC、SUA为负相关关系。
Objective To investigate the correlation between human serum 25 hydroxyvitamin D and peripheral vascular disease of type 2 diabetes.Methods The clinical data of 200 patients with type 2 diabetes who were treated in the Third People′s Hospital of Jingdezhen City in Jiangxi Province from December 2018 to January 2021 were retrospectively analyzed and were assigned to the research group.The patients were grouped according to their limb Doppler ankle brachial index(ABI).Patients with ABI≤0.9 were included in the disease group(n=100),patients with ABI>0.9 were included in the non-disease group(n=100),and 65 healthy subjects undergoing physical examination in the physical examination center during the same period were included in the control group.The clinical data and biochemical indicators[glycated hemoglobin(HbAlc),fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),homocysteine(Hcy),serum uric acid(SUA),25(OH)D]were compared between the three groups.Pearson correlation analysis on the correlation between 25(OH)D and peripheral vascular disease in type 2 diabetes was also carried out.Results The smoking rate,age,and body mass index in the disease group were higher than those in the non-disease group and the control group.The course of diabetes in the disease group is longer than that in the non-disease group.The prevalence of hypertension in the non-disease group was higher than that in the control group(P<0.05);there was no statistically significant difference in smoking rate and body mass index between the non-disease group and the control group(P>0.05).The HbAlc,FPG and TC in the non-disease group and the lesion group were higher than those in the control group.25(OH)D and HDL-C were lower than those in the control group(P<0.05);the 25(OH)D and HDL-C in the disease group were lower than those in the non-disease group,and SUA was higher than that in the non-disease group(P<0.05).The ratio of 25(OH)D level<20.0 ng/ml in the disease group was higher than
