摘要
目的探讨H19基因多态性与主动脉夹层发生风险的关系。方法本研究共纳入410例样本,包括主动脉夹层组146例,对照组264例。主动脉夹层组为2017年1月至2018年3月在南京医科大学第二附属医院和武汉同济医院影像学确诊为主动脉夹层的患者。对照组为同期在南京医科大学第二附属医院就诊的非主动脉夹层患者。收集纳入研究人群基础与临床资料,筛选H19基因及其启动子区tagSNPs。采取TaqMan技术进行基因分型,分析SNPs不同基因型与主动脉夹层发生风险的相关性。结果主动脉夹层组的吸烟率高于对照组(P<0.01)。主动脉夹层组男性患者和女性患者分别为111例(76.0%)和35例(24.0%)。在超显性模型中,rs217727与主动脉夹层风险性相关。与rs217727 GG基因型相比,rs217727 GA基因型可以增加主动脉夹层的风险(OR=1.39,95%CI 1.02~1.89,P=0.035),在显性模型中,与rs2177727 GG型相比,GA/AA型有增加主动脉夹层发生的趋势(OR=1.33,95%CI 1.00~1.79,P=0.052)。结论H19rs217727可能与主动脉夹层发生风险相关。
Objective To investigate the relationship between H19 gene polymorphism and the risk of aortic dissection.Methods A total of 410 subjects were used in this study.They comprised 146 patients with aortic dissection and 264 controls.The aortic dissection group consisted of patients diagnosed with aortic dissection in The Second Affiliated Hospital of Nanjing Medical University and Wuhan Tongji Hospital from January 2017 to March 2018.The control group consisted of non-aortic dissection patients who were treated in The Second Affiliated Hospital of Nanjing Medical University during the same period.The basic and clinical data were obtained from the included study population,and the H19 gene and its promoter region tagSNPs were screened.Genotyping with TaqMan technology was used to analyze the correlation between different genotypes of SNPs and the risk of aortic dissection.Results The number of patients who smoked in the aortic dissection group was higher than that in the control group(P<0.01).There were 111 male patients(76.0%)and 35(24.0%)female patients in the aortic dissection group.In the super-dominant model,rs217727 was associated with risk of aortic dissection.Relative to rs217727 GG genotype,rs217727 GA genotype increased the risk of aortic dissection(OR=1.39,95%CI 1.02-1.89,P=0.035).In the dominant model,compared with rs2177727 GG genotype,GA/AA type,tended to increase the incidence of aortic dissection(OR=1.33,95%CI 1.00-1.79,P=0.052).Conclusion These results suggest that H19 rs217727 may be associated with the risk of aortic dissection.
