摘要
目的探讨肺结核患者外周血辅助性T细胞9(Th9)的数量,及其分泌的白细胞介素9(IL-9)水平与病情程度、预后的关系。方法选择2019年4月—2020年4月甘孜医院收治的肺结核患者88例为病例组,同期健康体检88人作为对照。根据行业标准评价患者的严重程度,用酶联免疫吸附法测定血清IL-9水平,流式细胞仪测定外周血Th9细胞;受试者工作特征(ROC)曲线评估外周血Th9/IL-9对肺结核的诊断价值,采用多因素Logistics回归分析探讨预后相关因素。结果病例组Th9、IL-9水平及Th9/IL-9依次为(1.51±0.09)%、(25.13±1.54)ng/L、(6.31±0.15)%,高于健康体检人员(P<0.05),且随着病情加重,Th9、IL-9及Th9/IL-9水平逐渐升高(P<0.05)。ROC曲线分析显示,Th9、IL-9及Th9/IL-9的ROC曲线下面积分别为0.809、0.685和0.901,Th9、IL-9及Th9/IL-9诊断肺结核的灵敏度分别为82.9%、68.7%和91.5%,特异度分别为78.6%、60.5%和86.8%。多因素Logistics分析显示,Th9、IL-9及Th9/IL-9高表达均是影响肺结核患者预后的危险因素(P<0.05)。结论肺结核患者外周血Th9/IL-9水平明显升高,且随病情加重而升高。Th9/IL-9对于肺结核患者的诊断及预后具有较高的应用价值。
Objective To investigate the relationship between the levels of helper T cell 9(Th9),interleukin 9(IL-9)in peripheral blood and the disease severity,prognosis of patients with pulmonary tuberculosis.Methods A total of88 cases of pulmonary tuberculosis admitted to Ganzi Hospital from April 2019 to April 2020 were enrolled as the case group,with 88 individuals who underwent physical examination in the same hospital during the same period as the control group.The severity of disease was assessed according to relevant standards.The level of IL-9 was measured by ELISA,the number of Th9 cells in peripheral blood was measured by flow cytometry.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of Th9/IL-9 in peripheral blood for pulmonary tuberculosis.A multivariate Logistic regression analysis was used to analyze the factors affecting prognosis.Results Compared with the control group,the case group had significantly increased levels of Th9[(0.15±0.01)%vs(1.51±0.09)%],IL-9[(15.92±1.48)ng/L vs(25.13±1.54)ng/L],and Th9/IL-9[(0.94±0.04)%vs(6.31±0.15)%](P<0.05).The levels of Th9,IL-9 and Th9/IL-9 were significantly increased with the disease progress(P<0.05).The ROC curve analysis showed that the areas under the ROC curves for Th9,IL-9 and Th9/IL-9 were 0.809,0.685and 0.901,respectively,with the sensitivity for diagnosis of pulmonary tuberculosis was 82.9%,68.7%and 91.5%,respectively,and the specificity was 78.6%,60.5%,and 86.8%,respectively.The Logistic regression analysis showed that increased Th9,IL-9 and Th9/IL-9 were the risk factors affecting the prognosis of pulmonary tuberculosis(P<0.05).Conclusion The patients with pulmonary tuberculosis had a significantly increased level of Th9/IL-9 in peripheral blood,which was increased with the severity of the disease.Th9/IL-9 showed high value for the diagnosis and prognosis of pulmonary tuberculosis.
