摘要
目的分析糖链抗原19-9(CA19-9)、细胞角蛋白片段21-1(CYFRA21-1)、血管内皮生长因子(VEGF)检测在肺癌诊断及病情评估中的意义。方法2017年3月至2021年9月收治的116例肺癌患者(肺癌组),同期住院的77例肺部良性结节患者(对照组)。比较两组CA19-9、CYFRA21-1及VEGF水平。分析肺癌组不同分期患者CA19-9、CYFRA21-1及VEGF水平变化情况,并绘制受试者工作特征曲线(ROC)分析上述因子检测对肺癌的诊断价值。结果肺癌组CA19-9、CYFRA21-1及VEGF水平均高于对照组(P<0.05)。肺癌不同分期患者CA19-9、CYFRA21-1及VEGF水平比较,Ⅰ期<Ⅱ期<Ⅲ期(P<0.05)。CA19-9、CYFRA21-1及VEGF检测的最佳临界值分别为37 U/ml、3.3 ng/ml、125 pg/ml,三者联合检测的曲线下面积(AUC)、敏感度、特异度分别为0.917、0.941、0.852,均高于各项指标单独检测(P<0.05)。结论肺癌患者中CA19-9、CYFRA21-1及VEGF存在异常表达,通过检测上述因子水平可有效评估患者病情,且联合检测可提高肺癌诊断价值。
Objective To analyze the significance of detection of carbohydrate antigen 19-9(CA19-9),cytokeratin fragment 21-1(CYFRA21-1),vascular endothelial growth factor(VEGF)in the diagnosis and disease evaluation of lung cancer.Methods One hundred and sixteen patients with lung cancer(lung cancer group)admitted to our hospital from March 2017 to September 2021 and 77 patients with benign lung nodules(control group)hospitalized in our hospital during the same period were selected.The levels of CA19-9,CYFRA21-1,and VEGF were compared between the two groups.The changes of the levels of CA19-9,CYFRA21-1,and VEGF in patients with different stages of the lung cancer group were analyzed,and receiver operating characteristic curve(ROC)analysis was performed to analyze the diagnostic value of the above mentioned factors for lung cancer.Results The levels of CA19-9,CYFRA21-1 and VEGF in the lung cancer group were significantly higher than those in the control group(P<0.05).Comparison of the levels of CA19-9,CYFRA21-1 and VEGF in patients with different stages of lung cancer showed that stage I<stage II<stage III(P<0.05).The ROC curve analysis showed that the best cut-off values for CA19-9,CYFRA21-1,and VEGF were 37 U/ml,3.3 ng/ml,and 125 pg/ml,respectively.The area under thecurve(AUC),sensitivity,and specificity for the combined detection of the three indexes were 0.917,0.941,and 0.852,respectively,which were higher than those detected by each index alone.Conclusion There are abnormal expressions of CA19-9,CYFRA21-1,and VEGF in patients with lung cancer.The detection of the levels of these factors can effectively assess the patient′s condition,and the combined detection can improve the diagnostic value of lung cancer.
