摘要
目的探讨血清癌胚抗原(CEA)、细胞角蛋白片段19抗原21-1(CYFRA21-1)及胸苷激酶(TK1)在非小细胞肺癌早期诊断中的临床意义。方法选择2016年1月至2018年1月收治的非小细胞肺癌患者97例为观察组;另选择2016年1月至2018年1月收治的肺部良性疾病患者60例作为对照组。采用免疫放射分析法测定CEA含量,采用免疫放射分析法测定CYFRA21-1含量,采用点印迹免疫酶化学发光法测定TK1含量。比较2组血清CEA、CYFRA21-1和TK1水平变化,不同临床分期和淋巴结转移血清CEA、CYFRA21-1和TK1水变化。结果观察组血清CEA、CYFRA21-1和TK1水平高于对照组,差异有统计学意义(P<0.05);Ⅲ~Ⅳ期血清CEA、CYFRA21-1和TK1水平高于Ⅰ~Ⅱ期,差异有统计学意义(P<0.05);淋巴结转移血清CEA、CYFRA21-1和TK1水平高于无淋巴结转移,差异有统计学意义(P<0.05);CEA、CYFRA21-1和TK1联合诊断灵敏度和特异度高于单项检测。结论血清CEA、CYFRA21-1和TK1联合诊断非小细胞肺癌早期具有重要意义,值得临床借鉴。
Objective To investigate the clinical significance of serum carcinoembryonic antigen (CEA), cytokeratin fragment 19 antigen 21-1 (CYFRA21-1) and thymidine kinase (TK1) in the early diagnosis of non-small cell lung cancer (NSCLC). Methods Ninety-seven patients with NSCLC who were admitted and treated in our hospital from January 2016 to January 2018 were enrolled as observation group,and the 60 patients with benign pulmonary diseases who were admitted and treated in our hospital from January 2016 to January 2018 were enrolled as control group. The content of CEA and CYFRA21-1 was determined by immunoradiometric assay respectively. The content of TK1 was determined by dot blot immunochromatography. The changes in levels of serum CEA, CYFRA21-1 and TK1 were compared between the two groups, and the above indicators. The changes of serum levels of CEA, CYFRA21-1 and TK1 were observed and compared in patients with different clinical stages and lymph node metastasis. Results The serum levels of CEA, CYFRA21-1 and TK1 in observation group were significantly higher than those in control group ( P <0.05). The serum levels of CEA, CYFRA21-1 and TK1 in patients at stage Ⅲ~Ⅳ were significantly higherthan those of patients at stage Ⅰ~Ⅱ( P <0.05). The serum levels of CEA, CYFRA21-1 and TK1 in patients with lymph node metastasis were significantly higher than those pf patients without lymph node metastasis ( P <0.05). The diagnostic sensitivity and specificity of CEA, CYFRA21-1 and TK1 combination detection were higher than those by single index detection. Conclusion The combination detection of serum levels of CEA, CYFRA21-1 and TK1 is of great significance in early diagnosis of NSCLS, which is worthy of clinical reference.
