摘要
本文探讨颅脑超声在新生儿颅内出血诊断中的应用价值。选取接受颅脑超声检查的610例新生儿,分析颅脑超声诊断颅内出血的价值,同时分析不同分级脑室内出血患儿大脑前动脉、中动脉收缩期峰值流速(PSV)、舒张末期血流速度(EDV)及阻力指数(RI)差异。颅脑超声诊断颅内出血的灵敏性、特异性、准确性、阳性预测值和阴性预测值分别为92.13%、97.09%、96.39%、85.44%和98.62%;Papile分级Ⅲ~Ⅳ级脑室内出血患儿大脑中动脉PSV和EDV明显低于Ⅰ~Ⅱ级(P<0.05),而RI明显高于Ⅰ~Ⅱ级(P<0.05);Papile分级Ⅲ~Ⅳ级和Ⅰ~Ⅱ级脑室内出血患儿大脑前动脉PSV、EDV和RI比较差异无统计学意义(P>0.05)。大脑中动脉参数PSV、EDV和RI预测Ⅲ~Ⅳ级脑室内出血的ROC曲线下面积分别为0.750、0.917和0.776(P<0.05)。颅脑超声在新生儿颅内出血诊断中有较好的价值,同时大脑中动脉血流参数预测重度脑室内出血有一定应用价值。
This paper investigated the application value of craniocerebral ultrasound in the diagnosis of neonatal intracranial hemorrhage.Chosen 610 newborns who underwent craniocerebral ultrasound,and analyzed the value of brain ultrasound in the diagnosis of intracranial hemorrhage.The differences of peak systolic velocity(PSV),end diastolic velocity(EDV)and resistance index(RI)of anterior and middle cerebral arteries in children with different grades of intraventricular hemorrhage were analyzed.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of craniocerebral ultrasound in the diagnosis of intracranial hemorrhage were 92.13%,97.09%,96.39%,85.44%and 98.62%,respectively.The PSV and EDV of the middle cerebral artery in children with intraventricular hemorrhage in Papile gradesⅢ-Ⅳwere significantly lower than those in gradesⅠ-Ⅱ(P<0.05),while the RI was significantly higher than in gradesⅠ-Ⅱ(P<0.05).There was no significant difference in PSV,EDV and RI of anterior cerebral artery in children with papile gradeⅢ-ⅣandⅠ-Ⅱintraventricular hemorrhage(P>0.05).The areas under the ROC curve of middle cerebral artery parameters PSV,EDV and RI in predictingⅢ-Ⅳintraventricular hemorrhage were 0.750,0.917 and 0.776,respectively(P<0.05).Craniocerebral ultrasound has a good application value in the diagnosis of neonatal intracranial hemorrhage,and the blood flow parameters of middle cerebral artery have a certain application value in predicting severe intraventricular hemorrhage.
