癫痫持续状态患者血清NSE、TNF⁃α及GAL水平与脑电图异常程度的关系 被引量:4

Relationship between serum NSE,TNF⁃αand GAL levels and abnormality of electroencephalogram in patients with status epilepticus

摘要 目的分析血清神经元特异烯醇化酶(NSE)、肿瘤坏死因子⁃α(TNF⁃α)及甘丙肽(GAL)在癫痫持续状态患者中的表达与脑电图异常的关系。方法分析2017年3月至2019年4月巴中市中心医院癫痫持续状态患者136例临床资料设为观察组,另选65例同期体检健康者为对照组。分析两类患者NSE、TNF⁃α与GAL水平差异。同时对上述指标与脑电图异常的关系进行观察;并根据观察组2年随访结果分析不同预后患者NSE、TNF⁃α与GAL水平。结果观察组发作2、24 h时的NES、TNF⁃α与GAL水平明显高于对照组,差异有统计学意义(P<0.05)。经脑电图检查显示:观察组轻度异常患者15例、中度69例、重度52例;不同患患者NES、TNF⁃α与GAL水平指标水平比较:重度异常组>中度异常组>轻度异常组,差异有统计学意义(P<0.05)。脑电图与NES、TNF⁃α与GAL水平相关性分别为(r=0.767,0.311,0.372,P均<0.005)。观察组预后良好者60例、不良者76例;分析其就诊时发作2 h内、24 h后NES、TNF⁃α与GAL水平预后良好者明显低于预后不良者;发作2h内预后良好、预后不良者NES水平与发作24h比较差异有统计学意义(P<0.05)。结论癫痫持续状态患者血清NES、TNF⁃α与GAL水平与脑电图存在明显正相关关系,且可能参与患者疾病预后。 Objective To anlyze the relationship between the expressions of serum neuron⁃specific enolase(NSE),tumor necrosis factor⁃α(TNF⁃α)and galanin(GAL)in patients with status epilepticus and abnormal EEG.Methods The clinical data of 136 patients with status epilepticus in Bazhong Central Hospital from March 2017 to April 2019 were analyzed and set as the observation group,and 65 healthy people who underwent physical examination during the same period were selected as the control group.The differences in the levels of NSE,TNF⁃αand GAL between the two groups of patients were analyzed.At the same time,the relationship between the above indicators and abnormal EEG was observed;and the levels of NSE,TNF⁃αand GAL in patients with different prognosis were analyzed according to the 2⁃year follow⁃up results of the observation group.Results The levels of NES,TNF⁃αand GAL in the observation group at 2 and 24 hours after seizure were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The EEG examination showed that there were 15 cases of mild abnormalities,69 cases of moderate abnormalities and 52 cases of severe abnormalities in the observation group.Comparison of NES,TNF⁃αand GAL levels in different patients:the severe abnormality group>the moderate abnormality group>the mild abnormality group,the difference was statistically significant(P<0.05).The correlations between EEG and NES,TNF⁃αand GAL levels were 0.767,0.311,0.372(P<0.05).There were 60 cases with good prognosis and 76 cases with poor prognosis in the observation group;analysis of the levels of NES,TNF⁃αand GAL within 2 hours and 24 hours after the onset of the onset of the patients with good prognosis was significantly lower than that of those with poor prognosis.The level of NES in patients with good prognosis and poor prognosis within 2 hours of attack was significantly different from that within 24 hours of attack(P<0.05).Conclusion Serum NES,TNF⁃αand GAL levels in patients w
