急诊早期应用呼吸机抢救急性重症有机磷农药中毒致呼吸衰竭的临床观察 被引量:5

摘要 目的分析探究急诊早期应用呼吸机抢救急性重症有机磷农药中毒致呼吸衰竭患者的临床疗效。方法回顾性分析2018年6月至2019年7月进入我院接受抢救的84例急性重症有机磷农药中毒致呼吸衰竭患者的临床资料,采取随机盲选法把其平均分为两组,每组42例;其中对照组实行常规急诊治疗,观察组在此基础上实行早期呼吸机治疗,对比两组患者的临床疗效、临床指标、患者家属总体满意率、病死率及血气指标。结果对比统计结果发现,对照组的治疗有效率为33例(78.57%),与观察组的41例(97.62%)相比差异明显,两组数据具备统计学意义(P<0.05);对比统计结果发现,对照组的自主呼吸恢复时间、意识恢复时间及住院时间与观察组相比,差异有统计学意义(P<0.05);对比统计结果发现,对照组治疗后的pH、动脉血二氧化碳分压(PaCO_(2))及动脉血氧分压(PaO_(2))与观察组相比,差异有统计学意义(P<0.05);对比统计结果发现,观察组的患者家属总体满意率为40例(95.24%),与对照组的34例(80.95%)相比,差异有统计学意义(P<0.05)。结论急性重症有机磷农药中毒致呼吸衰竭实行早期呼吸机抢救治疗的效果显著,能明显提高治疗有效率,缩短自主呼吸恢复时间及住院时间,控制血气指标始终处于平稳状态,且提升患者家属总体满意率。 Objective To analyze and explore the clinical effect of emergency early use of ventilator to rescue patients with acute severe organophosphorus pesticide poisoning respiratory failure.Methods The clinical data of 84 cases of acute severe organophosphorus pesticide poisoning patients with respiratory failure admitted to our hospital from June 2018 to July 2019 were retrospectively analyzed.The random and blind selection method was evenly divided into two groups,with 42 cases per group.Among them,the control group received routine emergency treatment,and the observation group received early ventilator treatment on this basis.The clinical efficacy,clinical indicators,overall satisfaction rate of patients'families,mortality and blood gas indicators of the two groups were compared.Results Comparing the statistical results shows that,The treatment response rate of the control group was 33 patients(78.57%),compared with the 41 cases(97.62%)in the observed group.Data of the two groups had statistical significance(P<0.05).Comparing the statistical results shows that,the recovery time of spontaneous breathing,consciousness recovery time and hospitalization time in the control group were compared to the observation group,the difference was significant(P<0.05).Comparing the statistical results shows that,pH,arterial partial carbon dioxide pressure(PaCO_(2)) and arterial partial oxygen pressure(PaO_(2)) in the control group compared with the observed group,The difference has statistical significance(P<0.05).Comparing the statistical results shows that,the overall satisfaction rate of the patient families in the observation group was 40 patients(95.24%),compared with the 34 cases(80.95%)in the control group,the difference was significant(P<0.05).Conclusion Respiratory failure caused by acute severe organophosphorus pesticide poisoning early rescue breathing machine treatment effect significantly,can significantly improve the treatment efficiency,shorten the spontaneous breathing recovery time and hospitaization time,remain the
