摘要
目的:探究锥形束CT在安氏Ⅱ类2分类患者正畸前后髁突位置变化中的应用效果。方法:回顾性分析2018年4月~2020年2月本院收治的45例安氏Ⅱ类2分类患者的正畸前后临床资料,45例患者均伴有不同程度上的颞下颌关节症状。在正畸治疗前、治疗后给予患者进行锥形束CT检查,并测量其关节间隙、髁突情况、关节弹响、双侧髁突动度、开口型异常率。结果:在锥形束CT下可见,正畸治疗后患者关节前间隙小于治疗前,关节上间隙与关节后间隙大于治疗前,关节窝高度明显高于治疗前,差异显著(P<0.05);同时正畸治疗后患者的髁突纵距大于治疗前,差异显著(P<0.05),但髁突横距与髁突头高度比较差异不显著(P>0.05);治疗前患者关节弹响发生率、开口型异常率明显高于治疗后,双侧髁突动度明显高于治疗后,差异显著(P<0.05)。结论:安氏Ⅱ类2分类患者正畸前后应用锥形束CT,能明确判断患者的关节间隙、髁突情况与颞下颌关节症状改善情况,对临床制定正畸治疗方案以及监测正畸治疗效果具有重要意义。
Objective: To explore the cone beam CT in Angina’s Class Ⅱ 2 classification of condylar position changes before and after orthodontic patients application effect. Methods: The clinical data before and after orthodontic treatment of 45 patients with Angina’s Class Ⅱ and Class 2 who were admitted to our hospital from April 2018 to February 2020 were retrospectively analyzed. 45 patients with different degree of the temporomandibular joint symptoms. Conical beam CT was performed before and after orthodontic treatment, and the joint space and condylar process were measured. Joint ringing, bilateral condylar motion, and abnormal rate of opening type. Results: According to cone-beam CT, the anterior articular space after orthodontic treatment was smaller than that before treatment, the superior and posterior articular space were larger than that before treatment, and the height of the articular fossa was significantly higher than that before treatment(P<0.05). At the same time, after orthodontic treatment, the longitudinal distance of condyle was greater than that before treatment, and the difference was significant(P<0.05), but the difference between the transverse distance of condyle and the height of condyle head was not significant(P>0.05);The incidence of joint ejection and opening abnormalities was significantly higher than after treatment, and the bilateral condylar movement was significantly higher than after treatment(P<0.05). Conclusion: Angina’s Class Ⅱ 2 classification using cone beam CT before and after orthodontic patients, can clear judgment’s joint clearance, condylar of the patients with TMJ symptoms improve the situation, to clinical orthodontic treatment plan and monitoring the efficacy of orthodontic treatment is of great significance.
