摘要
目的评价在锥形束CT(cone beam compnted tomography,CBCT)图像上测量牙槽骨高度的准确性和可靠性。方法8个人类湿颅标本经CBCT扫描后测量图像上牙尖点到釉牙骨质界距离(T-CEJ)、牙尖点到牙槽嵴顶距离(T-BM)、釉牙骨质界到牙槽嵴顶距离(CEJ—BM)。在标本上实际测量以上项目。将两种测量值进行配对f检验。选择一定数量的位点重复测量,计算组内相关系数(ICC),进行一致性检验。结果CBCT测量值均略小于标本实际测量值。其中对T-CEJ的测量两种方法无统计学差异,差值均值为-0.041mm;对T-BM和CEJ-BM的测量两种方法有统计学差异,差值均值分别为-0.268mm和-0.208mm。所有重复测量的ICC均大于0.96。结论CBCT测量牙槽骨高度与标本测量虽然存在统计学差异,但差异较小,在临床实际可接受范围之内。CBCT测量的可靠性较好。
Objective To evaluate the accuracy and reliability of cone-beam computed tomography (CBCT) in measuring alveolar bone height. Methods Eight human cadaver head samples were scanned with CBCT technology. Measurements were made on each tooth from the cusp or the middle point of incisal margin to the cementoenamel junction (T-CEJ), from the cusp or the middle point of incisal margin to the alveolar bone margin (T-BM) and from the cementoenamel junction to the alveolar bone margin (CEJ-BM) along the long axis of the tooth. Measurements were made both in the CBCT images and on the real samples. The paired-t test was made between the two different measurements to evaluate the accuracy of the CBCT measurements. And 210 sites from 6 samples were measured. The Interclass correlation coefficient (ICC) was calculated to evaluate the reliability of CBCT measurements. Results The values of CBCT measurement were smaller than those of the direct measurement. Significant difference was found in the values of T-BM and CEJ-BM. The mean deviations were - 0. 268 mm and - 0. 208 mm. The values of T-CEJ showed no significant difference and the mean deviation was -0. 041 mm. The ICC of all the measurement was above 0. 96. Conclusions Significant difference was found in the measurement of alveolar bone height between CBCT and direct measurement. But the mean deviation was very small. The CBCT measurement showed high reliability.
