摘要
目的探究血清热休克蛋白47(HSP47)、转化生长因子-β1(TGF-β1)在慢性房颤并发早期心衰中的表达水平及诊断价值。方法回顾性分析本院2019年3月—2021年3月收治的慢性房颤患者75例,其中单纯慢性房颤23例、心功能Ⅰ级~Ⅱ级32例、心功能Ⅲ级~Ⅳ级20例,同期选取40例健康体检者作为对照组,比较4组血清HSP47、TGF-β1水平和左心房内径,并进行相关性分析。结果慢性房颤3组患者血清HSP47、左心房内径均显著高于对照组(P<0.01),血清TGF-β1显著低于对照组(P<0.01)。Pearson相关性分析显示,慢性房颤患者左心房内径与HSP47水平呈正相关(P<0.01),与TGF-β1水平呈负相关(P<0.01)。HSP47诊断慢性房颤并发早期心衰的敏感度(93.33%)、特异度(80.00%)和曲线下面积(0.947)均明显高于TGF-β1(89.33%、72.50%、0.887)(P<0.05)。结论血清HSP47、TGF-β1在慢性房颤并发早期心衰患者中均呈异常表达,对其进行动态监测有助于预测患者病情严重程度。
Objective To explore the levels and diagnostic value of serum heat shock protein 47(HSP47)and transforming growth factor-β1(TGF-β1)in chronic atrial fibrillation complicated with early heart failure.Methods A retrospective analysis was performed on 75 patients with chronic atrial fibrillation admitted to our hospital from March 2019 to March 2021,including 23 cases of simple chronic atrial fibrillation,32 cases of cardiac function gradeⅠ-Ⅱ,and 20 cases of cardiac function gradeⅢ-Ⅳ.At the same time,40 healthy subjects were selected as the control group,and the serum HSP47,TGF-β1 levels and the left atrial diameter of the four groups were compared,and the correlation analysis was also performed.Results The serum HSP47 and left atrial diameter of the three groups of chronic atrial fibrillation were significantly higher than those of the control group,and the serum TGF-β1 was significantly lower than that of the control group(P<0.01).Pearson correlation analysis showed that the left atrial diameter of patients with chronic atrial fibrillation was positively correlated with the level of HSP47(P<0.01),and negatively correlated with the level of TGF-β1(P<0.01).The sensitivity(93.33%),specificity(80.00%)and area under the curve(0.947)of HSP47 in diagnosing chronic atrial fibrillation complicated with early heart failure were significantly higher than TGF-β1(89.33%,72.50%,0.887)(P<0.05).Conclusion Serum HSP47 and TGF-β1 are abnormally expressed in patients with chronic atrial fibrillation complicated with early heart failure,and dynamic monitoring of them can help predict the severity of the patient’s condition.
