摘要
目的探讨血浆外泌体环状RNA(circRNA)hsa_circ_0022417在胃癌中的表达及其临床意义。方法选取临床明确诊断胃癌初诊患者60例,慢性胃炎患者30例(疾病对照组)和同期健康体检者30名(健康对照组),分别检测各组血浆外泌体中hsa_circ_0022417的表达水平以及血清CEA、CA19-9的表达。采用ROC曲线评价各项指标诊断胃癌效能。分析血浆外泌体hsa_circ_0022417的表达与胃癌患者临床病理参数的关系。结果胃癌组血浆外泌体hsa_circ_0022417的相对表达水平高于慢性胃炎组和健康对照组,差异有统计学意义[(1.89±0.15)比(0.93±0.08)比(0.75±0.07),F=9.96,P<0.05]。胃癌组织中hsa_circ_0022417的表达水平高于癌旁组织,差异有统计学意义[(2.78±0.24)比(1.18±0.11),t=6.08,P<0.05],并且其表达水平与血浆外泌体表达水平呈正相关(r=0.72,P<0.05)。ROC曲线分析结果显示,血浆外泌体hsa_circ_0022417、血清CEA、CA19-9单指标诊断胃癌的曲线下面积分别为0.79、0.68和0.66,3种指标联合检测诊断胃癌的曲线下面积为0.86。血浆外泌体hsa_circ_0022417的相对表达量与肿瘤大小(χ^(2)=6.42,P<0.05)、分化程度(χ^(2)=5.83,P<0.05)、临床分期(χ^(2)=7.14,P<0.01)及淋巴结转移(χ^(2)=5.17,P<0.05)均有关。结论外泌体hsa_circ_0022417在胃癌患者血浆中高表达,对临床胃癌辅助诊断和早期筛查具有重要意义。
Objective To investigate expressions and clinical significance of plasma exosomal hsa_circ_0022417 in gastric cancer(GC).Methods Sixty gastric cancer patients,30 chronic gastritis patients(disease control group)and 30 healthy volunteers(healthy control group)were enrolled in this study.The expression levels of plasma exosomal hsa_circ_0022417 and serum CEA and CA19-9 were detected.The ROC curve and AUC were used to estimate the diagnostic capacity.Results Compared with chronic gastritis patients and healthy control,the expression of plasma exosomal hsa_circ_0022417 was significantly upregulated in the gastric cancer group(F=9.96,P<0.05).The expression level of hsa_circ_0022417 in GC tissue was significantly higher than that in adjacent tissue(t=6.08,P<0.05).The AUC of hsa_circ_0022417,serum CEA and CA 19-9 was 0.79,0.68 and 0.66,respectively.The combined detection of three indicators had the highest AUC(0.86)(P<0.05).The expression level of exosomal hsa_circ_0022417 was significantly correlated with tumor size(χ^(2)=6.42,P<0.01),differentiation degree(χ^(2)=5.83,P=0.05),TNM stage(χ^(2)=7.14,P<0.05)and lymph node metastasis(χ^(2)=5.17,P<0.05).Conclusion Exosome hsa_circ_0022417 is highly expressed in the plasma of GC patients,which is of great significance for clinical auxiliary diagnosis and early screening of GC.
